Job Overview: We are seeking a compassionate and dedicated Services Specialist to join our team. The Services Specialist will be responsible for coordinating and delivering essential services to seniors, program facilitation, advocacy, and resource coordination.
Key Responsibilities:
- Resource Coordination:
- Provide information and assistance on available community resources, benefits, and support services.
- Coordinate referrals to external agencies and healthcare providers as necessary.
- Advocate for seniors' rights and access to services within the community.
- Program Facilitation:
- Collaborate with the program team to develop and implement engaging services, workshops, and events for seniors.
- Facilitate group sessions and educational programs on topics relevant to aging, health, and wellness.
- Evaluate program effectiveness and gather feedback from participants to inform continuous improvement.
- Documentation and Reporting:
- Maintain accurate and confidential client records, including assessments, care plans, and progress notes.
- Compile data and prepare reports on service utilization, outcomes, and program impact.
- Ensure compliance with regulatory standards and reporting requirements.
- Team Collaboration:
- Work collaboratively with colleagues, volunteers, and community partners to support a holistic approach to senior care.
- Participate in team meetings, case conferences, and professional development activities to enhance skills and knowledge.
- Contribute to a positive and inclusive work environment that values diversity and teamwork.
Qualifications:
- Bachelor's degree in social work, gerontology, human services, or related field.
- Previous experience working with older adults and knowledge of aging-related issues.
- Strong communication, empathy, and interpersonal skills.
- Ability to work independently, prioritize tasks, and problem-solve effectively.
- Proficiency in Microsoft Office Suite and electronic record-keeping systems.
