Enliven Home Health is an organization providing comprehensive care for Columbus area residents, enabling them to remain in their own homes and assisted living communities as they recover from injury or illness, or approach end-of-life.
Our nursing staff has the unique opportunity to meet patients where they live, and collaborate with patients and families, their physicians, and a strong interdisciplinary team helping clients achieve their personalized plan of care. In addition to providing direct patient care, the RN Case Manager plans, organizes and directs home care services including PT, OT, ST, and home health aides to ensure needs and goals are met.
Responsibilities and Duties:
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
- Develops and implements a care plan, which establishes goals based on nursing diagnoses and incorporates therapeutic, preventive, rehabilitative, or palliative nursing actions. Includes the patient and the family in the planning process.
- Completes routine nursing visits to provide skilled assessment, education, wound care, obtain lab specimens, and perform other ordered treatments.
- Maintains accurate and timely documentation
- Communicates with physicians, interdisciplinary team, facility staff, and leadership as appropriate with a focus on patient-centered care
- Compliance with Federal/State guidelines, rules, regulations, statutes, and law.
- Completes nursing visits in homes and assisted living facilities within the Columbus area

Requirements:
· Active Ohio RN license
· Strong interpersonal and communication skills
· 2-3 years nursing experience preferred
· Home health experience preferred
· Proficiency with technology and experience with EMRs
· Reliable vehicle, current driver’s license, and auto insurance

Pay: $75,000.00 - $85,000.00 per year
