Diabetes Nurse Educator $32.00/hr. to $46.00/hr. Full-time position
Niche clinic service Native Americans and Alaska Natives in the greater Chicagoland area needs quality nursing staff to provide exceptional patient care!
Summary: The Diabetes Nurse shall be a currently licensed Registered Nurse who is certified as a Diabetes Health Nurse and Educator. He/she shall work within our Medical Health Department providing outreach and education services to the local community and Native population.
- Coordinates all services and activities of the SDPI Program to expand services and increase public awareness of diabetes which includes maintaining data management system, evaluate projects for programmatic performance, update and work with sub-grantee in carrying out grant objectives.
- Oversees SDPI budget and ensures grant application and all financial and monthly reports are submitted to appropriate agencies and maintain necessary documentation for financial and program records
- Supervision of the SDPI Program employees and oversight of the subgrantee staff, plans work schedules, identifies appropriate training and competencies, and manages HR duties for employees within the SDPI program
- Provide education and teach patient and families on diabetes management.
- Must be willing to work with grants and do data reporting for the grant.
Qualifications: 1. Demonstrates the ability to work in a culturally diverse environment. 2. Licensed in IL. A master's degree in nursing is highly desired. 3. Minimum 2 years' experience in Diabetes nursing preferred. 4. Minimum 1 year in a medical clinic. 5. Exhibits the ability to manage time and possesses organizational skills, excellent communication and personal skills. 6. Is proficient with software applications such as Excel and/or other data base programs, Microsoft Word, Internet, E-mail, and Power Point.
Requirements: 1. Must travel to the communities in the service area surrounding Cook County and immediate suburbs. 2. Must be available for out of the area travel as required for job related training and CEU's, and to attend all required meetings. 3. Managerial competence to: (1) must be able to work independents or with a team; (2) develop and manage a departmental budget; 4. Must provide documentation of immunity to measles, and rubella or become immunized with the recommended vaccine and also Hepatitis C. Must test annually for TB and have an annual health exam. Must be immunized annually for flu. 5. Must possess valid driver's license, good driving record. 6. Must adhere to confidentiality and HIPAA policies. 7. Must successfully pass a pre-employment drug screening test and be willing to submit to a criminal background check. Must have Grant Knowledge and Experience.
