Hospice & Community Care of Rock Hill, SC is actively hiring an Admissions Nurse (RN) to work Full Time!
Shift Details: Monday through Friday 8:00am-4:30pm
Hospice & Community Care is seeking an Admissions Nurse (RN) to work Full Time to serve our community in and around the Rock Hill, South Carolina region.
What You’ll Do:
- Plan for admission of patients in coordination with hospitals, physicians, community agencies and other referral sources.
- Assess potential patients for hospice criteria. Responsible for establishing customer relationships, collecting pertinent data, and presentation of agency services.
- Admit patients, develop initial plan of care in accordance with the patient’s physician, and complete required paperwork.
- Make visits to patients within the service area to provide nursing care, instruction, and support to patients and their family.
- Other duties as assigned.
What You’ll Need:
- Registered Nursing license in South Carolina, or be eligible for licensure in South Carolina.
- Valid CPR certification
- Valid Driver’s License and reliable transportation
- Experience in healthcare, home care, or hospice setting is preferred, but not required
