Overview:
We are seeking a skilled and experienced MDS Coordinator to join our team. As an MDS Coordinator, you will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments for our patients. This is a critical position that requires strong attention to detail and knowledge of patient care, hospice care, and medical coding.
Responsibilities:
- Conduct comprehensive assessments of patients' medical records and care plans
- Ensure accurate and complete documentation of patient information, including medical history, diagnoses, treatments, and medications
- Collaborate with healthcare professionals to gather necessary information for MDS assessments
- Analyze patient data to determine appropriate care plans and interventions
- Monitor and update patient records as needed
- Stay up-to-date with changes in Medicare regulations and guidelines
- Provide education and training to staff members on MDS documentation requirements
- Participate in quality improvement initiatives to enhance patient care
Qualifications:
- Bachelor's degree in nursing or related field (preferred)
- Valid nursing license (RN)
- Minimum of 2 years of experience in a healthcare setting, preferably in a nursing home or hospital
- Strong knowledge of Medicare regulations and MDS assessment process
- Proficiency in medical coding in PCC
- Excellent attention to detail and organizational skills
- Ability to work independently and as part of a team
