SALARY INFORMATION:
- $36.66 - $40.01 Dependent on experience may vary outside this range
- Additional $2.00 - $4.00 an hour for additional weekend and night shifts
- Bonus (If Eligible)

QUALIFICATIONS:
- Current Colorado LCSW licensure
PREFERRED QUALIFICATIONS:
- Experience in Social work in a Clinical setting preferred.
- Master’s Degree or equivalent preferred.
BASIC FUNCTION: The Licensed Clinical Social Worker, as part of the Primary Care treatment team, identifies, triages and manages patients with behavioral health problems within our primary care clinic. They will typically report to the head of a unit/department. In addition, the LCSW will provide skill training through psycho-education and patient education strategies and will develop specific behavioral change plans for patients.
RESPONSIBILITIES:
- Works with primary care team to treat and manage patients with emotional and/or health problems.
- Assists in the detection of at-risk patients and development of plants to prevent further psychological or physical deterioration.
- Interviews clients and their families and coordinates and plans programs and activities to meet their social and emotional needs.
- Provides psychotherapy or counseling to individuals, groups, couples, or families.
- Evaluates patient care plans with primary care team.
- Teaches patients, families and staff about care, prevention and treatment enhancement techniques.
- Is responsible for ensuring that the medically related emotional and social needs of the patients are met and maintained on an individual basis in accordance with policies and procedures and current federal, state and local standards, guidelines and regulations.
- Performs a variety of tasks and other duties as assigned.
QUALIFICATIONS:
- Current Colorado LCSW licensure
PREFERRED QUALIFICATIONS:
- Minimum of 2 to 4 years’ Social Work experience in a Clinical setting preferred.
- Master’s Degree or equivalent preferred.
