Experience Requirement: We are looking for candidates with a minimum of 1 years of nursing experience. Those with prior HOME HEALTH EXPERIENCE will be given preference.

Position Summary: As a Home Health Nurse (Case Manager) with Safe Life HHC, your role will take you into the heart of our patients' homes. You'll be responsible for delivering multifaceted care, ensuring they maintain their independence, and devising care plans tailored to each patient's unique diagnosis and requirements. Your duties will range from administering IVs at home, managing wounds, to keeping physicians updated about their patient's well-being.
Key Responsibilities:
- Comprehensive patient assessment covering physical, psychological, and social aspects.
- Document patient observations and conditions during each visit.
- Curate a personalized care plan for each patient and ensure adherence.
- Educate patients or caregivers on essential self-care skills for home management of chronic conditions.
- Oversee medication protocols and management.
- Specialized care including wound and IV management, PICC Line, G-Tube, and Foley care.
- Collaborate with a diverse team, from physicians to therapists, ensuring an integrative care approach.
Qualifications:
- Possession of, at a minimum, an Associate's Degree in Nursing or a Bachelor of Science degree in Nursing.
- Home Health experience and familiarity with its documentation are HIGHLY DESIRED.
- Capability to work autonomously or synergistically with a medical or therapeutic team.
- Sound understanding of Home Health care regulations and best practices.
- Versatility and empathy to operate in diverse living conditions.

Pay: $80,000.00 - $100,000.00 per year
