JOB SUMMARY: As the lead of our SAMSHA grant funded GBHI program, this role will oversee the day-to-day operations of the GBHI program services, supervise program staff, and monitor program deliverables. The program provides helps communities expand and strengthen treatment and recovery support services for individuals (including youth and families) experiencing homelessness who have substance use disorders or co-occurring mental and substance use disorders. Our program also assists with Housing resources.
WORK EXPERIENCE:
Minimum 2 years of nonprofit program supervisory experience
Minimum 3 years experience providing services to individuals with substance use, mental illness and homelessness.
Minimum 2 years of management experience
Minimum 2 years of housing program experience
LICENSURE: LMSW/LMHC licensure is required.
DUTIES AND RESPONSIBILITIES:
- Manage the implementation of the GBHI program, including outreach to build census and oversight of program staff and deliverables.
- Supervise the identification, stratification, and prioritization of the homeless population to identify individuals in need of behavioral health services.
- Oversee the assessment of patient's medical needs, behavioral health, substance use, functional abilities, as well as concrete needs such as housing, finances, medical insurance, food, clothing, legal, educational, vocational, social and family support, language and immigration issues, and barriers/access to care and services.
- Collect baseline and ongoing data for the program (outcome assessment) and review or create individualized service plans with the patient, recognizing the patient’s priorities and role as a decision-maker.
- Supervise the provision of concrete and clinical referrals, referral monitoring, and outreach, including street outreach.
- Counsel patients using Motivational Interviewing, Harm Reduction, Trauma-Informed Care, and Contingency Management.
- Work closely with program staff to remove barriers to patient success by maintaining regular and frequent communication, conducting case conferences, managing crises, and troubleshooting issues.
- Participate in CQI meetings, and program meetings, and solicit feedback from program participants.
- Oversee the collection of post-program patient data and identify opportunities for process and procedure enhancements to drive efficiency and customer service levels.
- Promote program services and engage the target population.
- Provide all services in a culturally and linguistically competent manner, encouraging direct client participation in ongoing program implementation and management.
- Conduct intakes, assessments, and treatment plans, as well as assign cases and tasks to staff as needed.
- Complete all monthly and other required reports.
- Perform other related duties as assigned.
REQUIRED SKILLS:
Advocacy, communication, team building, and knowledge of Evidence-Based Practices (motivational interviewing and contingency management preferred).
