Position Summary: We are seeking a dedicated and experienced Virtual Nurse Practitioner to join our Hospice Care team. In this role, you will provide high-quality, patient-centered care to hospice patients remotely, working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our patients and their families.
Key Responsibilities:
- Patient Care:
- Conduct virtual consultations and assessments with hospice patients and their families.
- Develop and implement individualized care plans in collaboration with the hospice care team.
- Monitor patient progress and adjust care plans as necessary to ensure comfort and quality of life.
- Provide symptom management, palliative care, and end-of-life care in accordance with best practices and patient wishes.
- Collaboration and Communication:
- Work closely with physicians, nurses, social workers, and other healthcare professionals to coordinate comprehensive care.
- Maintain clear and consistent communication with patients, families, and the hospice care team.
- Participate in regular interdisciplinary team meetings to discuss patient care and progress.
- Documentation and Compliance:
- Document all patient interactions, care plans, and treatments accurately and in a timely manner.
- Ensure compliance with all federal, state, and local regulations, as well as organizational policies and procedures.
- Keep up-to-date with the latest developments in hospice and palliative care through continuous education and professional development.
Qualifications:
- Education and Licensure:
- Degree in Nursing from an accredited program.
- Current and unrestricted Nurse Practitioner license in the state of practice.
- Certification in Hospice and Palliative Care preferred.
- Experience:
- Minimum of 2 years of experience as a Nurse Practitioner, preferably in hospice or palliative care.
- Experience in providing virtual care or telehealth services is highly desirable.
- Skills and Abilities:
- Strong clinical skills in hospice and palliative care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficient in using electronic health records (EHR) and telehealth platforms.
- Compassionate and empathetic approach to patient care.
Working Conditions:
- This is a remote position, allowing for flexibility in work location.
- Requires reliable internet access and a quiet, private workspace for conducting virtual consultations.
- May involve occasional on-call responsibilities and flexible hours to accommodate patient needs.
