Lifestyles Coordinator:
Job Summary: Assists the Lifestyles Director with managing the day-to-day life enrichment operations of the community to ensure resident’s programming and activities needs are met. Ensures residents are treated with respect and dignity and ensures they are being engaged in appropriate activities as outlined in their personal service plan.
Reports to: Lifestyles Director
Essential Functions:
1. Responsible for the direct implementation of resident-based programming and activities in the community.
2. Provides training, supervision, and monitoring of associates in the administration of planned and unplanned activities with residents.
3. Provides dementia training and education to resident care associates on an ongoing basis with classroom in-services, and situation-specific training when needed.
4. Performs ongoing assessment/observation of residents’ physical and cognitive needs and coordinates with the Wellness Director and Lifestyles Director to ensure personal service plans are up to date and accurate.
5. Assists in initiating individualized service plans, proactively manages life enrichment activities and services for each resident, evaluates effectiveness and maximizes the resident's opportunity to remain in their environment through resident centered programming.
6. If needed, participates in pre-admission screening of prospective new residents. Assures that required documentation is completed prior to or upon resident admission related to life enrichment
7. When needed, assists Resident Wellness Director when needed with ongoing assessment/observation of residents' life enrichment needs and coordinates with other departments to assure quality, proactive care.
8. Evaluates residents; documents resident’s change in condition, and notifies resident wellness director, executive director, physician, and resident’s responsible party of those changes.
9. Participates in or leads meetings relevant to life enrichment issues, such as Service Plan meetings, involving appropriate parties as needed.
10. Participates in department quality improvement activities.
11. Assists in keeping the environment safe for associates to reduce the occurrence of Workers’ Compensation claims by appropriately evaluating resident transfer and involving therapy services through resident wellness director and/or executive director.

Education and Experience: Basic typing skills along with a working knowledge of personal computing and word processing software are required, preferably in a Microsoft Windows environment.

Physical Demands: It is understood that this a physically demanding job and involves lots of walking and does include the lifting and transferring of residents when appropriate.
