RN Staff Nurse (Part-time/Full time) - Adult Day Health Care (ADHC)
Duties: The Silicon Valley Adult Day HealthCare is a therapeutically oriented outpatient day program that provides health maintenance, rehabilitative services, socialization, case management, and caregiver support/education and day respite, in a congregate setting to geriatric participants who are functionally impaired and are often the frail elderly.
As a Staff Nurse you will provide a full range of nursing care to participants both in person and over the phone. Our facility is seeking individuals who have experience in geriatrics or gerontology, with knowledge of dementia and possess good organizational skills and follow through, a demonstrated ability to assume leadership in a patient centered program, and the ability to establish effective communication with participants, families, caregivers and members of the interdisciplinary team.
Work Schedule: Full-time/Part-time
Salary: $40 and up per hour
Job duties include, but are not limited to, the following:
1. Responsible for providing nursing services, including treatment plans, initial and on-going assessments addressing medical, physical, nutritional and functional needs of the patients and reporting any significant changes to primary care provider; coordinate any on-site medical or health care emergencies.
2. Assures detailed, current and comprehensive documentation is contained in the patient medical record and other pertinent records, which includes, but is not limited to, intake information, progress notes, health and behavioral issues, care plans
3. Participates in and initiates Quality Assessment and Improvement program activities as applicable to the clinical area and contributes to the development of policies, procedures and/or clinical guidelines.
4. Assures adherence to strict safety procedures and Infection Control Standards by all staff and continuously monitors the environment for potential and/or actual safety hazards.
5. Provide patient and/or caregiver education re: diagnosis, disease process, symptoms and treatments, dementia care best practices and caregiver stress. 6. Supervise LVN's, RNA's, NA's, activities and voluntary staff. Provide input into staff evaluations and training of staff.
Qualifications & Evaluations
Basic Requirements:
*CA RN license
*Documentation to work in the U.S.
* Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
* Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration AND degree equivalency documentation will meet the requirement of graduation from an approved school of professional nursing.
* Mandarin-speaking is a plus.
