Job Summary:
We are seeking a detail-oriented and experienced MDS Coordinator to join our Briarwood Health Center by Harborview team. The MDS Coordinator will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments, as well as ensuring compliance with all state and federal regulations. The ideal candidate will have strong organizational skills, excellent attention to detail, and a thorough understanding of the MDS process.
Duties:
- Coordinate and complete the MDS assessments for all residents in accordance with state and federal regulations
- Collaborate with interdisciplinary team members to gather necessary information for accurate assessment completion
- Review medical records, care plans, and other documentation to ensure accuracy and completeness of assessment data
- Ensure timely submission of MDS assessments to meet regulatory requirements
- Participate in care plan meetings to discuss resident needs and develop appropriate care plans
- Provide education and training to staff regarding the MDS process and documentation requirements
- Monitor changes in resident conditions and update assessments as needed
- Maintain knowledge of current regulations and guidelines related to the MDS process
Qualifications:
- Valid certification as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
- Previous experience as an MDS Coordinator in a healthcare setting preferred
- Strong understanding of MDS assessment process and regulations
- Proficiency in using electronic medical record systems, such as Cerner
- Knowledge of occupational health, gastric lavage, DME, specimen collection processing, family planning, memory care, pediatrics, and laboratory procedures is a plus
- Excellent attention to detail and ability to maintain accurate records
- Strong communication skills, both verbal and written
