As a member of the UpliftedCare's Interdisciplinary Team (IDT), the hospice nurse is a key person in identifying the holistic needs of the terminally ill patient and family. This is accomplished by planning, implementing and evaluating nursing care through a systematic assessment of health needs and optimum utilization of personnel. The nurse provides skilled nursing care according to the Plan of Care established by the IDT, providing symptom control and comfort with emotional support to all those involved facilitating death with dignity.
