What we're looking for:
- Bachelor's Degree in Social Science or related fields
- Proficient in Microsoft office and computer skills
- Excellent Oral and Written Communication Skills
- Ability to work Non- traditional hours
- State of Alabama Adult Case Management Training or attend training
- Knowledge of Community Lay-out
- Knowledge of Community Agencies and Resources
- Personal Automobile Liability Insurance in the amounts of $100k, $300k, $50K
- Must have a valid drivers license with a clean record for 5 years
- Must attend Homeless Management Information System (HMIS) training
- must attend on-line training for SSI/SSDI, Outreach, access and recovery
Synopsis of Duties:
Attend Continuum of Care (CoC) meetings as point person
Work in frequent contact with supervisor and community stakeholders to assist clients
Visit clients in their environment to assess progress towards goals per schedule
Serve as primary case manager to duplex of 4 apartments (8-person caseload)
Serve as primary case manager for Evidenced-Based Supportive housing (12-person caseload)
Serve as auxiliary case manager for Shelter-Plus Program (62-person caseload)
Complete apartment inspections as required by HUD standards
Report, follow up, and document apartment concerns with supervisor and facility manager
Facilitate individual/group basic living skill training sessions
Serve as Representative Payee as appropriate
Maintain up-to-date payee program documentation
Complete documentation within 24 hours per policy
Assist clients with scheduling in-house and community appointments
Develop and maintain open communication with team, clients, and their support system
Provide appointment reminders to clients and their support system
Demonstrate an understanding of Shelter-Plus, Evidenced-Based and Sunrise Garden programs
Respond to client crisis as necessary
Attend Soup Kitchen as scheduled (non-traditional hours)
Assist with locating safe and affordable housing for homeless clients
Input data into Homeless Management Information System (HMIS)
Utilize SSI/SSDI, Outreach, Access, and Recovery (SOAR) application for homeless clients
Complete other duties as assigned by supervisor
