HIRING CLIENT ENGAGEMENT AND NAVIGATION SERVICES (CENS) NAVIGATOR
Location: Downey / Santa Fe Springs (may be co-located)
Position: Client Engagement and Navigation Services (CENS) Navigator
Salary: Determined based on experience, credential, skillset and education.
Summary: Reporting to the CENS Program Supervisor and/or CENS Director, the CENS Navigator helps guide potential clients through the process of accessing services, and with their consent, schedules appointments, maintains open communication with the referring agencies and assigned SUD provider, and follows up to make sure linkage to care is completed. The CENS Navigator also provides direct clinical services to clients participating in treatment with, or enrolled in programming offered through, the Client Engagement and Navigation Services department.
Essential Job Functions:
- Provide clients with basic information regarding the Specialty Substance Use Disorder system;
- Conduct SUD screening using the ASAM Triage tool;
- Determine whether client may have a substance use disorder and the most appropriate provisional level of care for treatment;
- Ensure the client is successfully linked to the referred providers;
- Assist clients with service navigation and linkages to needed SUD treatment services, as well as ancillary services including, but not limited to, mental health, physical health, legal aid and CalFresh benefits;
- Provide CENS services in accordance with the most current version of the provider SAPC Substance Use Disorder Treatment Manual;
- Work collaboratively with Managers, Coordinators and other staff as needed;
- Facilitate a referral to an appropriate SUD provider;
- Assist clients in navigating the SUD system of care;
- Engage clients at CENS co-located sites or in the field at frequent and regular intervals to gain their trust and motivate them to accept SUD treatment services;
- File documents and maintain charts in accordance with established agency practices and procedures;
- Prepare documentation in appropriate system of record and file same in accordance with established agency practices and procedures;
- Assist in determining Medi-Cal or My Health LA eligibility;
- Comply with confidentiality and HIPAA requirements;
- Comply with mandated reporting requirements;
- Familiarity with all agency practices, policies and procedures, including but not limited to CARF requirements, and compliance with same;
- Provide timely and professional responses to internal and external communications;
- Participate in continuing education, as required by Certifying bodies and/or agency;
- Attend and participate in regular, on-going in-service trainings for the benefit of the agency;
- Participate in staff meetings as required;
- Attend meetings with applicable governing agencies and funding sources;
- Interact with County and State agencies, as required;
- Perform community outreach and development of referral sources;
- Familiarity with the agency’s EOC (Environment of Care) plan;
- Other duties as requested by the CENS Program Director and/or Supervisor.
Qualifications:
- Must be registered or certified SUD counselor, LPHA or License-eligible LPHA;
- For staff working with Youth, have at least two years of experience working with youth;
- Have completed or will complete within one month of hiring, trainings on the ASAM Triage Tool, Youth engagement screener, CBT, MI, and EHR Data entry;
- CENS staff must follow the requirements outlined in DPH-SAPC’s latest Provider Staffing Guidelines bulletin.
- For work at certain co-locations, CENS staff may be required to successfully complete a heightened background check to obtain clearance.
Education:
- High School Diploma and SUD Certification, SUD registration and enrollment in SUD Education or related college degree.
Working Conditions:
- Work is primarily performed in an office.
- Transportation to appointments for clients is required, as needed.
- Some outdoor activities may be required.
- Work is in a busy environment in the CENS call center.
- Some work will be co-located out of various L.A. County facilities, including: Los Angeles County Probation Department in East Los Angeles and Rio Hondo; Courthouses in Downey, Bellflower, Norwalk and East Los Angeles; Family Solution Center at The Whole Child; Department of Public Social Services CalWORKs and General Relief, and Adult At-Risk; DCFS SUD TIPS; LEAD – Alma Family Services; Permanent Supportive Housing sites in Los Angeles, East Los Angeles, Commerce, Bell and Huntington Park; and Interim Housing.
Must be able to pass the following pre-employment screenings:
- Background check, including criminal history (LA CADA considers all applicants equally and will take into account each applicant’s individual life circumstances).
- Drug and health screen, including negative TB test
- Employment and education verification
