Overview:
We are seeking a compassionate and skilled Part-Time Registered Nurse (RN) to join our team. As a part of the Senior Helpers team, you will play a vital role in assessing needs, developing and implementing care plans, and coordinating services to ensure our clients receive the best possible care in their homes. This position offers a flexible schedule primarily between the hours of 9am-5pm (as well as outside business hours if desired).
Responsibilities:

Conduct comprehensive nursing assessments on new and existing clients as the face of the company.
- Develop and implement individualized care plans in collaboration with Care Manager and Client Service Manager
- Educate clients and their families on disease management and home safety measures.
- Document all care provided in a clear and concise manner according to agency and regulatory standards.
- Maintain strong communication with Care team, families, and other healthcare professionals involved in the client's care.
- Work collaboratively with the home care team to ensure seamless coordination of services.
- Adhere to all company policies and procedures, including infection control standards.
- Provide non-medical/skilled care to clients
Qualifications:

Active RN license.
- Minimum of 4 years of experience in home care or a related setting.
- Experience working with VA and/or Medicaid
- Strong clinical skills and assessment abilities.
- Excellent communication and interpersonal skills.
- Ability to work well with different types of client management software via mobile apps and laptops (Microsoft360 and WellSky experience is a bonus)
- Ability to work independently and as part of a team.
- Time management and organizational skills.
- Valid driver's license and reliable transportation.
