Appalachian Mountain Health ("AMH"), a Federally Qualified Health Center network based in beautiful Asheville, NC, with clinics in Asheville, Murphy, Sylva, and Robbinsville, NC, is seeking to hire a Licensed Clinical Social Worker/ Behavioral Health Therapist to join our exceptional clinical team in delivering quality-focused primary care and behavioral health services at our Brand-New Murphy Clinic.
AMH provides comprehensive, accessible, patient-centered primary care to all, including low-income, medically underserved and uninsured populations, at six locations throughout Western North Carolina. Position Summary:
The Licensed Integrated Behavioral Health Provider is responsible for providing outpatient mental health resources for patients within the primary care setting. The Integrated Behavioral Health provider will work collaboratively with a team of medical and mental health providers. Responsibilities of this role include short term mental health counseling to Appalachian Mountain Health patients to help them build resiliency and internal resources to cope with stressors in their lives.
Essential Duties:
· Maintain open communication and accessibility to encourage mental health integration in the clinic setting.
· Interviews individuals to assess needs, as well as develops appropriate response plans for their clients.
· Performs Bio-psychosocial assessments.
· Work with a variety of patients with a wide range of mental health diagnoses, and lived experiences, including substance use disorders.
· Provides information and referrals for counseling and service acquisition as appropriate to the assessed need.
· Provides consultation services for mental health recommendations to patient’s treatment team.
· Initiates treatment interventions; e.g., individual, and family counseling and crisis interventions as appropriate.
· Support patients through difficult circumstances with an emphasis on building sense of internal locus of control.
· Provide patient education.
· Chart all patient interaction, referrals and maintains notes within 36 hours in compliance with CMS documentation standards.
· Protects confidentiality and the abide by HIPAA Laws as well as 42 CFR.
Educational Requirements:
· Master's Degree in Social Work
· Master’s Degree in Mental Health Counseling
· 3+ years in a Clinical Setting
· Licensed Clinical Social Worker
· Licensed Clinical Mental Health Counselor
