About Your Work
Join our strong, seasoned and mission-driven Inpatient Care Management team as a Care Coordinator where you'll work in tandem with our clinicians and provider partners who are passionate about patient-centered care and patient and provider experience, and where you will be responsible to support the team for our members who are hospitalized and at their most vulnerable. Your role will involve the provision of administrative and clerical support to the Inpatient Care Management team and our Provider Partners in the community.
As a Care Coordinator you'll work closely with our interdisciplinary team of nurses, physicians, Medical Directors, and the health care team at the hospital who provide and manage the care and transitions for our members. We seek individuals with a strong commitment to patient-centered care and a passion for promoting health and wellness, and your work will be guided by compassion, integrity and accountability.
In this role, your primary focus is to support the receipt, entry and tracking of data related to hospital admissions and continued stay, production and management of census reports. You will support the Inpatient Care Management nurses in the coordination of care and in ensuring the accuracy and timeliness of data to patient record.
You Will Focus On:

Obtaining, entering accurately, and tracking inpatient authorizations and records, information related to continued stay and discharge.

Perform coordination of census reporting and the gathering additional information by contacting hospitals, facilities, providers.

Assisting the Inpatient Care Management nurse team in the coordination of care for transitions and discharges

Preparation, issuance and retention of determination letters, and maintenance of timely and accurate documentation.

Communicate with Health Plans, care management or prior authorization teams for coordination of care.

Generation of referrals and authorizations for appropriate resources.

Answering phone, email inquiries from our hospital or skilled nursing facilities, our Provider Partners, Health Plans or our internal departments.
The Skills, Experience & Education You Bring

Experience and proficiency in medical terminology and coding.

Accurate data entry and documentation in the core authorization system.

Familiarity with and experience in managed care concepts and practice.

Advanced computer skills and experience using Microsoft software suite products.

Detail-oriented and ability to document timely and accurately and in accordance with department policies and procedures.

Creative, innovative and adept at problem solving and solutions-oriented approaches.

Excellent verbal communication skills; Customer focus- Internal and External.

Education- High School Diploma, some college or AA preferred. Some familiarity and experience in health care field; working knowledge medical terminology.

Minimum two years previous experience in administrative support.

The anticipated pay range for this role is listed in our salary posting for transparency but may vary based on factors including the candidate’s qualifications, skills, experience, and geographic location. This base pay range is specific to California and can vary based on Bay Area, Metro LA, and Greater California regions which may not be applicable to other locations.

Physical Requirements: Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork – Activity level: Sedentary, frequency most of workday.
