General Description of the position:
The Nurse is to promote the health and wellbeing of each resident by providing accurate nursing assessments, giving appropriate input to the development of care plans, educating management staff on the appropriate and proper care of each resident, delegates and assigns nursing tasks, and is a team player.
Licensed nurses must deliver the following nursing services:
Knowledge of Oregon Administrative Rules
Monitoring of resident condition when the resident has been placed on Alert Charting
Participation on Service Planning Team.
o If the resident experiences a significant change of condition and the service plan is updated, the licensed nurse must participate on the Service Planning Team or must review the service plan with date and signature within 48 hours.
Health Care Teaching and Counseling. A licensed nurse must provide individual and group education activities as required by individual service plans and facility policies.
Intermittent Direct Nursing Services, if a resident requires nursing services that are not available through hospice, home health, a third-party referral, or the task cannot be delegated to facility staff, the LPN and/or RN will provide those services up to 7-days.
Monitor the medication & treatment administration program.
Documentation for each resident, when needed
Perform evaluations concerning new move-ins, 90-day service plan reviews, safe medication distribution, falls, smoking and restraint use.
Review staff’s ability to administer medication safely.
Review resident’s ability to administer medication safely, if that is their desire.
Supervise staff/residents/others who have been delegated and/or assigned nursing tasks.
Perform selected clinical tasks that reflect an unusual or unique condition in a resident or that cannot be delegated (IM injections)
Wound care
Assess the health condition and health care needs of the resident with complex medical needs, prior to admission, and determine if admission is appropriate.
In cooperation with the Administrator, establish the appropriate level of care for the resident.
Serve as liaison between facility, applicant’s family and appropriate medical personnel. (Assist facility to problem solve when change in resident’s health status occurs.
Coordinate with other community health care professionals, such as the home health agency, to acquire use of their expertise when necessary.)
Monitor status of resident’s health condition and initiate actions in response to changing needs.
o Provide staff training for establishing proper plan of action and documentation if an injury occurs.
o Oversee and ensure completion of state-required documentation and forms
o Extraordinary tasks:
Finger sticks for diabetics
Peri-care
Handling soiled linen.
Clean up of potentially contaminated fluids.
Attending to an injury where blood is present.
I understand that because of the above-mentioned routine tasks, I could be at potential risk for contracting a bloodborne pathogen, including but not limited to non-A hepatitis, non-B hepatitis, hepatitis B (HBV), delta hepatitis, syphilis, malaria and/or human immunodeficiency virus (HIV).
Therefore, the company will provide me with the appropriate training in Universal Precautions and OSHA ‘s Exposure Control Standards in order to minimize any possible exposure. They will further provide me with an HBV vaccination if I so choose.
It will be my responsibility to adhere to all Exposure Control Policies as set forth by the company I have read the above and have received the training on Bloodborne Pathogens.
Working conditions:
Sitting/Standing: up to 8 hours
Lifting requirements: up to 200 lbs. with assistance.
Travel: position does require attendance at certain training sessions and/or retreats. Mileage will be reimbursed at the current government standards. Hotel, food and airfare will be paid by the Company
Phone time: up to 4 hours daily

Must have excellent communication skills both written and verbal, with Residents, their families, staff and Management Company and/or Owners
Equipment used:
Office equipment including but not limited to telephones, computers, copiers and printers
Occasional tasks may require the use of (but not limited to) lifts and needles.
Qualifications:
Knowledge of basic scientific principles necessary to provide competent nursing care.
Ability to assess human responses to actual or potential health problems.
Understanding of the normal dosage, action and common side effects of most medications in common use.
Ability to utilize the principles of teaching and learning, which are employed in both resident and staff teaching.
Genuine concern for and ability to work with the elderly.
Ability to train and supervise personnel.
Ability to lead and motivate people.
Ability to involve others in the decision-making process while maintaining responsibility for all decision.
Ability to manage and prioritize a large, complex workload.

Graduate of an accredited school of professional nursing.
