We are presently seeking
a Care Coordinator. Requirements include:
Requirements: High School Diploma or GED required; Community Health Worker certification preferred; 2 years of experience working in a Community Health setting preferred; Knowledge of Medicare/ Medicaid and other insurance plans preferred; Experience working with the Medicaid/ Medicare population preferred.
The Care Coordinator position will primarily work with specific target populations to educate patients on disease states, engage patients in care, and coordinate care plan objectives for the purposes of improving patient care and health outcomes. Coordinates with clinical practice teams through all phases of the care continuum. The Care Coordinator position will work with both internal and external healthcare entities to coordinate health services with individuals, families, and the community. Expedites the continuum of care to achieve coordinated high-quality care that is patient centered.
