As a Hospice Registered Nurse You Will
- Provide skilled professional nursing care to patients.
-Be available to work on weekends On-call
-Report on-call activities to the RN Triage Nurse throughout and at the end of the call shift.
-Make patient visits to wherever the patient calls home; as indicated by need.
-Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and effective grieving for patient and family.
- Oversee and provide primary care, including all clinical care, symptom control and identify other physical, psychological, social and spiritual needs for assigned patients.
-Regularly assess the patient for nursing needs.
-Implement the Plan of Care Provisions to meet their needs, initiate appropriate intervention and support for the patient and family upon admission and re-evaluate the patient’s nursing needs to provide appropriate, comprehensive and responsive care plan interventions.
- Work closely with other members of the interdisciplinary team to ensure all patient and family care needs are being met.
Required Skills Qualifications
-Current Registered Nurse licensure.
-Associates or Bachelor’s degree.
-Minimum of one year general nursing preferred with current Hospice, home health, medical, surgical or critical care experience.
-Experience with pain and symptom management interventions.
-Ability to work within an interdisciplinary setting.
-Ability to work a rotating schedule that includes weekend and holidays.
-Reliable transportation and current insurance.
-Electronic medical record experience.
-Must be able to provide own transportation to various locations in service areas as required by duties.
