We are hiring Care Coordinators (CC) also known as BHTS, MHTs, Peer support, to manage and maintain the safety of the patients. Care Coordinator's are viewed as the heartbeat of a multidisciplinary team that provides direct inpatient care to adults 18+ who are challenged with mental health and/OR substance use disorders.
- Building rapport with patients, clinical staff and medical staff.
- Work as a One Team Mindset
- Transporting patients to and from any outside appointments when needed
- Observing patients and ensure safety while in treatment.
- Assist clinical and medical staff with intakes, orientations and daily operations.
- Assisting in Self-Administered Medication
- Conducts and documents individual's vitals and reports all questionable results to medical staff. Conducts and documents all BAC and urine drug screens
- Documents all services and significant individual interactions in the electronic health record. Monitors and documents patients every 15 mins
- Responsible for maintaining and ensuring the cleanliness of the facility. Performs light cleaning of interior center, restocking and launders individuals clothing when needed
- Facilitates group sessions daily to individuals
- Demonstrates positive role modeling for the clients through mentoring and coaching while utilizing recovery skills
