Summary of Position
This position performs administrative and clinical duties under the Director of Nursing and in collaboration with the licensed provider. Must be able to maintain continuity of care, efficient patient flow and function in high productive clinic. Must be able to interview patients to obtain history, vital signs, assist with treatments, conduct point-of-care testing, and have knowledge of EMR, patient scheduling, Vaccines for Children and CHDP. This position shall comply with the organizations mission, goals and policies.

Standard: Works with the care team to ensure that each patient has the best possible experience in our health center:
- Introduces self and informs patients of their providers name, who will render services as part of the PCMH team – manages up, reassuring patient that we will provide the best possible care.
2. Assures timeliness of services rendered to patients and gives patient frequent updates on predicted duration of wait times.
- Able to maximize patient flow and keep up with provider by ongoing assessment of the provider schedule and working with other staff to minimize patient wait time.
4. Provides information regarding medical treatment and/or services to be offered during the visit and explains them to the patient/parent.
- Assists with the provision of cultural and linguistic competent communication and translates as needed.
6. Maintains positive communication with all staff and superiors
- Listens to complaints from patients regarding services rendered and provides assistance in the resolution of complaints including the notification of management
Standard: Demonstrates the necessary clinical skills to provide services to our patients at the top of the Medical Assistant Scope of Practice in California.
1. Performs direct patient care as identified by Licensed Vocational Nurse Scope of Practice.
2. Demonstrates basic nursing skills necessary to implement provider’s orders and clinical interventions (vital signs, height/weight, tobacco use, etc.)
- Performs age appropriate screening as needed or as directed by provider
4. Maintains competency in all point of care laboratory procedures, including but not limited to: urine dipstick, glucose monitoring, urine pregnancy testing, rapid strep, lead level, hemoglobin.
5. Prepares treatment rooms and patients for examination and ensures exam room/area well stocked with supplies
- Demonstrates accurate knowledge in management of emergency situations (able to set up oxygen, get assistance, call 911)
7. Able to carry out and maintain competency in standing orders.
8. Under the direction of the licensed provider, administers injections and other diagnostic tests/referrals as ordered, including immunizations, completing lab and x-ray requisitions, and other services ordered by the provider
9. Observes the patient’s reaction to medication/immunizations administered
10. Able to reconcile immunizations with yellow immunization card, CAIR and eCW
- Collects specimens for testing, labels specimens accurately and ensures appropriate transport of specimens with appropriate and completed requisitions.
12. Reviews patient’s electronic health record for accuracy and completes outstanding orders.
13. Able to record accurate information obtained from patient or parent/guardian in the electronic health record in a timely manner
- Able to create and accurately document telephone encounters with patients
Standard: Provides care coordination to patients on care team panel
- Performs pre-visit planning to ensure that all ordered labs, imaging, consult reports, discharge summaries have been obtained in advance of patient appointment with provider
2. Provides information regarding medical treatment and/or services to be offered during the visit and explains them to the patient/parent.
3. Ensures that clinical visits summaries are printed and provided to each patient at the conclusion of the visit
4. Prints out relevant health education materials as ordered by the provider and ensure the patient receives the information with an explanation of the materials.
5. Answers calls from pharmacies, or responds to faxed requests from pharmacies and appropriately routes refill requests to providers
6. Tracks lab and imaging tests until results are available.
7. Assists with follow up of referrals and/or abnormal results in collaboration with licensed provider.
8. Performs follow up action on failed appointments
9. Assists provider and other members of the care team in managing the assigned panel of patients as part of the WMCHC Population Health Program.
- Is responsible for notifying patients of recommended health care services as directed by policy and procedure (i.e. pediatric immunizations, cervical cancer screening, mammogram, colon cancer screening)
b. Is able to explain importance of routine screening tests and able to provide patients with written information.
- Helps reinforce and explain the Care Plan to patients.
11. Helps reinforce and explain the importance of Self- Management.
12. Able to follow up with licensed providers regarding medical records reviews, patient forms etc.
Standard: Actively participates in the Quality Assurance and Improvement Plans for the WMCHC.
- Follows policy and procedures on inventory, ordering and facility maintenance
2. Updates and enters accurate information into required logs
3. Assists in preparation of office for a site review and audit
- Ensures safety of patients and visitors by adhering to all safety rules and regulations
5. Maintains and protects PHI and privacy in accordance with WMCHC polices and HIPAA
6. Listens to complaints from patients regarding services rendered and provides assistance in the resolution of complaints including the notification of management
- Participates in QI committees as assigned.

- Is an effective team member who is flexible, cooperative and willing to take initiative in assisting others.
2. Attends and actively participates in departmental meetings, PCMH team meetings and huddles.
Qualifications:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
Education:
- High School Diploma or equivalent (GED)
- Medical Assistant Diploma – accredited vocational school
Licensures/Certification:
- CMA Certified through AAMA Preferred
- BLS/CPR – American Heart Association
Knowledge/Skills:
- Must be fluent in Spanish/English
