Eastern Carolina Housing Organization (ECHO) is a non-profit homeless coalition which encompasses the 13 northeastern counties of South Carolina (SC) and 3 counties in North Carolina (NC). The main corporate office is located in Myrtle Beach, SC with a satellite offices being located in Florence, SC and Sumter, SC.
Overall Role/ Responsibilities:
The role of the Health Care Navigator is to provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties. Health Care Navigators work closely with the client's primary care provider and members of the client's assigned interdisciplinary treatment team. The Health Care Navigator will act as a liaison between ECHO and community medical clinics and work with a population of clients with complex needs who require assistance accessing health care services or adhering to health care plans. The Health Care Navigator works closely with the client's assigned multidisciplinary team, including medical, nursing, and administrative specialists, and case management personnel. The Health Care Navigator works within this team to provide timely, appropriate, client centered care equitably. The Health Care Navigator works collaboratively with the team and the client to identify and address systems challenges for enhanced care coordination as needed. The right person for this position will be empathetic, open-minded, detail oriented, and excited about helping the people experiencing homelessness that ECHO is working with. This job requires some travel to all of the counties in the ECHO service area. Program-specific paperwork and record-keeping will be required. It is vital that the right person for the job has time-management skills, mastery of organization, and the ability to communicate compassionately, and educate the Veterans on their health care options and related benefits
Essential Functions of Position:

- Complete non-clinical assessment to understand the client's situation, potential barriers to care, the causes, and the impact of such barriers on the client's ability to access and maintain health care services
- Develop case plans, participate in the delivery and connection to supportive services, and maintain a case load
- Assist clients in communicating their preferences in care and personal health-related goals to facilitate shared decision making of the client's care
- Serve as a resource for education and support for clients, and provide education services and materials that match the health literacy level of the client
- Regularly review care plan goals with the client, conduct regular non-clinical barrier assessments, and provide resources and referrals needed to support adherence
- Periodically evaluate the effectiveness of the resources and referrals provided and make appropriate modifications to ensure the provision of high-quality care and interventions
- Monitor client's progress, maintain comprehensive documentation, and provide information to treatment team members when appropriate
- Assist client in identifying concerns or questions about their treatment or medications to develop open communication with the provider or treatment team
- Provide comprehensive case management and care coordination across episodes of care – act as a health coach by proactively supporting the client to optimize treatment interventions and outcomes
- Be willing to travel the agencies service area (upper eastern section of South Carolina and North Carolina)
- Input case notes and client information into database
- Maintain compliance with funding or regulatory entities by maintaining case files, case notes, and all program records
- Adhere to professional boundaries and ethics in the care of others (e.g., critical incident reporting, HIPPA, Duty to Warn) 14. Work as part of a team to ensure best practice models for helping the homeless are adhered to
Qualifications:
Minimum Qualifications
· Bachelor’s Degree and 2 years’ experience in a health care or social services field preferred
· Ability to follow written and oral directions
· Valid driver’s license and reliable transportation
· Spoken and written fluency in English and an ability to communicate effectively with diverse populations, including organization staff, medical professionals, and other service providers in the community, as well as the homeless population that we serve
· Ability to work flexible hours to fulfill position duties
· Effective time-management and attention to detail
· Ability to maintain confidentiality of client information, including health and wellness information
· Be able to learn quickly and not be afraid to accept constructive criticism
· Ability to work with clients in a compassionate, non-judgmental manner
· Must be proficient using Microsoft Office (Word and Excel)
· Ability to think critically and problem solve efficiently while under pressure
· Be willing to be trained and operate in a trauma-informed manner
· Excellent organizational skills and the ability to manage multiple projects at once
· Ability to work as a member of a team and promote teamwork with other staff members
· A self-starting attitude is vital; must be comfortable working independently with clients in the office and in their home.
