JOB DESCRIPTION:
PHMC’s Pathways to Recovery Program is looking for a full-time Bilingual Case Manager to provide case management and psycho-education to program participants.
QUALIFICATIONS:
- Bachelor’s Degree and three (3) years experience in social/human services or Master’s Degree and one (1) year of related experience
- Bilingual in Spanish Preferred
- Expertise in case management and client advocacy
- Ability to work with addicted individuals with critical and emergent situa Sensitivity to cultural and economic differences
- Thorough knowledge of social services and resources available in Philadelphia
- Understanding of medical and psychosocial issues related to addiction
- Valid Pennsylvania Driver’s License and ability to drive a 15 passenger van
RESPONSIBILITIES:
- Perform client needs assessment, based on intake materials and client interviews
- Provide case management/care coordination and advocacy by assisting clients in negotiating social services, providing follow-up and monitoring support. Reassess client need as appropriate
- Maintain contact with client’s therapist and other supports as needed
- Complete continuing care plan with each client, two (2) weeks prior to discharge from treatment
- Attend weekly individual supervisory meetings with the Program Director
- Maintain accurate, timely records of time spent with client, problem areas, problem resolution, etc.
- Participate in staff development as required
- Provide client transport to social services and appointments as needed
- Coordinate and monitor client use of transpasses, facilitate Logisticare transportation
- Co-facilitate specialized client groups including conflict resolution, life skills and other duties as assigned
- Identify housing options for clients and coordinate admission to housing
- Other duties as assigned
TREATMENT PLAN:
- Provide input to clinical team
PROGRESS REPORT:
- Maintain continuous record of all contacts, events, and progress toward accomplishment of goals as stated in Treatment Plan
- Complete case management needs assessment within 3 days of admission
- Record individual, case management and group progress notes in the DAP format after each therapeutic activity
- Complete reviews in compliance with state licensing and other program guidelines
- Maintain charts that meet all standards
CONTINUING CARE PLAN:
- Assist treatment team and participants with the information of continuing care plans
- Other duties as assigned
SALARY:
- Commensurate with education and experience
- Grade 16
