Job Overview:
The SAFE Provider/MHCT Navigator works with SafeCare Augmented for Family Empowerment (SAFE) and the Medical Home Community Team (MHCT). SafeCare Augmented, is an evidence-based home visiting service (EBHV), with the purpose of empowering families, supporting parents, and promoting children's health, development and safety for Philadelphia families. SAFE Provider/MHCT Navigator provides parent training curriculum for parents of children 0-5 who are at risk for or have been reported for child neglect or physical abuse. SAFE Provider/MHCT Navigator will work with families in their homes or virtually to increase parents’ skills in the areas of parent-child interaction, home safety, and child health helping parents build a solid foundation for their children’s health and wellbeing. This SAFE Provider/MHCT Navigator will also be responsible for providing services through MHCT program with support in health education, systems navigation, and collaboration with families, medical practices and social service providers. Along with conducting MHCT assessments and health screenings, observe behavioral health, communicate with clients, and make appropriate referrals to community agencies and organizations for services not provided by the SAFE program. The SAFE Provider/MHCT Navigator must be willing to work a flexible schedule including some evening hours, as requested.
Responsibilities:
- Works closely with Program Coordinator to enroll and serve eligible families.
- Assists in new families' screening and orientation activities.
- Delivers SAFE services to parents in Home or Virtually.
- Administers assessments required by program.
- Provides and monitors services to families as outlined in the SAFE Model Implementation Training.
- Delivers SAFE activities, and curriculum that enhances parent education while being both creative and flexible.
- Attends all relevant planning meetings, team meetings, trainings, and other events.
- Remains open and responsive to coaching, supervision, and feedback.
- Works with Program Manager and team to complete all relevant reports.
- Performs other duties as assigned by the Program Coordinator and/or Program Manager.
- Enters client data via web-based applications.
- Establish flexible and after hour availability appointments when visiting parents.
Planning, Preparation and Documentation
- Monitors supplies needed for delivery of services and alerts supervisor when replacement supplies need to be ordered.
- Maintains accurate written and electronic records and documentation. Inputs data in required databases.
- Participates actively in all training programs required and attends additional training as requested by Program Coordinator or Program Manager.
- Reports suspected child maltreatment and/ or suspected child maltreatment as well as adult maltreatment and/ or suspected adult maltreatment when abuse is observed or suspected.
Patient Navigation Activites
- Carries a caseload of 10-20 cases of enrolled families.
- Conducts home/virtual visits to develop individual and community-based plans of care, provide health education and promotion services, and provide follow-up visits with families.
- Establishes and maintains linkages with community health, social service, education and legal service agencies, and other support services through effective communication between staff, community members and MHCT partners.
- Coordinates all aspects of family and individual health navigation and education.
- Conducts follow-up with referral sources.
- Provide education that includes a focus on parent-child interaction, home safety and medical wellness.
- Assesses client and family needs and strengths with input from other team members (nurse, other case managers, program supervisor) and family members.
- Schedules/coordinates patient/healthcare appointments, including: facilitating connection between patient and current primary care provider or linking patient to a new primary care provider; facilitating transportation to appointment, if needed; and accompanying patient to appointment, if needed.
- Utilizes/Provides interpretation services during appointments, if needed.
- Conducts appointment follow-up, if needed, including coordinating/scheduling follow-up services, and providing additional referrals for supportive services.
- Provides care coordination services to families with the direction of Program Manager.
- Meets regularly with Program Manager to provide family updates and communicate successes and anticipated barriers/challenges.
Skills:
- Excellent problem solving, conflict resolution, time management and professional communication (written and oral) skills.
- Comfortable delivering interventions to families in various home situations.
- Excellent interpersonal skills and ability to effectively interface with partner organizations, families, parents, children and community residents.
- Ability to establish priorities, and work both independently and in a team environment to meet objectives with minimal supervision.
- Open and responsive to coaching and constructive feedback.
- Understands the importance of program fidelity.
- Advocate for client and community strengths and needs.
- Ability to assess and triage social services quickly.
- Advanced proficiency in Microsoft office suite and various web-based platforms with an ability to learn new software, as needed.
- Must clear child abuse, criminal history check and FBI clearance.
- Ability to acquire information about new systems, organizations and practices.
- Motivational interviewing techniques.
Experience:
- At least two years experience working with diverse populations, low-income individuals, individuals from different backgrounds.
- Minimum of three years of direct in-home, community-based and/or clinical services.
- Experience in data collection/entry and evaluation monitoring.
Education Requirement:
- Bachelor's degree in social work, public health, or a related field.
Salary:
- Commensurate with education and experience.
- Grade 16.
