PURPOSE
The Registered Nurse (RN) provides and coordinates the care of patients/families and significant others in the ambulatory care setting by assessing, diagnosing, planning and intervening in actual or potential health problems, including evaluation of the patient’s response to the care. These responsibilities also include patient/family/significant other education and health maintenance strategies. The RN is a member of a designated health care team and collaborates, coordinates and guides team members in planning patient care and achieving goals.
The Registered Nurse demonstrates knowledge of the principles of human growth and development over the life span and the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his or her age, specific needs, and to provide the care needed as described by standards and policy and procedures.
DESCRIPTION OF COMPETENCIES
Clinical/Technical/Service
- Systematically and continuously assesses patient’s physical, psychosocial and spiritual health status and knowledge base to determine patient’s health care and educational needs.
- Conducts comprehensive nursing health assessment interview and medication verification.
- Perform clinical assessment.
- Provides and/or delegates patient care according to the care goals. Sets priorities based on patient need.
- Performs triage for scheduled and non-scheduled patients.
- Validates provider orders.
- Communicates pertinent information and changes in patient status to appropriate members of the care team and other health care providers.
- Collaborates with team members to make necessary changes in plan of care.
- Practices and monitors infection control.
- Coordinates client care including referral back to primary provider or other specialty.
- Assesses the need for additional referrals, VNS, Social Work, etc. and initiates as indicated.
- Documents according to standards of care.
- Responsible for telephone communication

- Communicates normal and abnormal test results in collaboration with provider including appropriate follow-up.
- Provides additional patient outreach including but not limited to post visit, missed visits, pre and post-surgery and disease specific information / education.
- Obtains prior authorizations for medications.
- Performs therapeutic treatments and procedures
- Assists with procedures.
- Administers oral, injectable and inhaled medications and vaccines.
- Provides wound care.
- Administers blood and blood products, specific to specialty areas.
- Performs treatments such as chemotherapy administration specific to area of specialty.
- Assists with procedures such as conscious sedation and others specific to area of specialty.
- Monitors and assesses patient’s response to treatments and procedures.
- Initiates respiratory interventions as determined by clinical assessment.
- Accesses and maintains central lines/ports for blood drawing and medication administration.
- Coordinates, delegates and/or provides all other patient care activities.
- Responsible for all unit enabling operations including patient flow and assignment / reassignment of clinical staff.
- Initiates CPR and emergency response when necessary.
- Coordinates in-patient admissions and ER referrals.
- Acts as a patient advocate
- Reinforces the importance of designating a health care proxy.
- Assists patients in navigating the complexities of the health care system and managing care transitions.
- Establishes therapeutic and supportive relationships with patients and family.
- Promotes and provides for patient safety.
- Educates
- Assesses patient/family learning needs, including level of comprehension and barriers to learning.
- Teaches, counsels and assists individuals and families to assume responsibility for prevention of illness and the promotion, maintenance and restoration of health.
- Provides education to patient / family about treatments, tests, preps and procedures.
- Provides information and instruction on home and self-care.
- Organizes and conducts group teaching.
- Evaluates outcomes through return demonstration and/or verbalization of understanding.
- Identifies available community resources and refers and assists patient / family in utilizing such resources.

EDUCATION REQUIRED
- Bachelor’s Degree required (requirement waived for currently employed RN who does not possess this degree).
- Graduate of an Accredited School of Nursing .
EXPERIENCES AND/OR SKILLS REQUIRED

LICENSES/CERTIFICATIONS REQUIRED
- Current NY State Registered Nurse License and Registration
- Basic Life Support required. ACLS / PALS preferred.
- Certification in area of specialty is highly recommended
