Regional Hospice is looking for a FULL TIME PEDIATRIC RN CASE MANAGER for our home hospice pediatric patients.

This role is full time (40 hrs/wk) and you will work primarily day hours, with some early morning or evening flexibility. The Pediatric RN Case Manager is responsible for the coordination and management of all pediatric care and other services for patients and their families. The Pediatric RN Case Manager works with the interdisciplinary team, the patient’s pediatrician, and other pediatric specialists to provide quality pain and symptom management, as well as support to the patient and family.
Qualifications
- Registered Nurse with a current license in the State of Connecticut REQUIRED.
- Bachelor of Science in Nursing degree REQUIRED/
- Minimum of three (3) years experience as a pediatric nurse in a hospital REQUIRED.
- Minimum of one (1) year experience in hospice, palliative care, or home care (adult or pediatrics) REQUIRED
- Pediatric case management experience desired.
- Advanced credentials in hospice care (CHPPN) preferred.
- Transportation required, including proof of auto insurance and a clear driving history.
- Excellent verbal, written and computer skills – ability to communicate effectively with patients, families, physicians, interdisciplinary team members and others.

The Pediatric RN Case Manager must possess good communication and organizational skills and be able to work in a variety of environments. The Pediatric RN Case Manager presents a professional attitude and appearance at all times while maintaining strict confidentiality of patient and agency information.

Pay: $88,000.00 - $95,000.00 per year
