Due to the significant growth in census over the past year, VNA is looking to add another member to the team. Acting as the RN Case Manager, the Clinical Coordinator is responsible for the in home care of palliative patients including identifying and coordinating care plan development, medication evaluations and skilled psychosocial assessment. Anticipate and assist with symptom management, including identifying pain and suffering. Participate in family support, teaching, discharge planning and continuity of care across sites of care, in collaboration with the primary care team. Participate in both formal and informal education for all disciplines related to Palliative Care practices. Coordinates members of a multi‑disciplinary team, (e.g., RNs, LVNs, HHAs, PTs, OTs, STs, MSWs, nursing students and newly employed RNs), as appropriate, and supervises patient plan of care through case conference and ethical decision-making.Experience/ Education/ Qualifications: Must have possession of a current license to practice as a nurse in the State of California. Current CPR certification. Bachelor’s degree in nursing preferred. Must have excellent knowledge of fundamentals of pain and symptom management. Excellent interpersonal and communication skills. Demonstrated organizational skills. Must have the ability to establish and maintain cooperative relationships with professionals and community groups. Ability to work collaboratively as part of an interdisciplinary team.
