PRIMARY FUNCTION:
The primary duties of the Patient Navigator are to coordinate the care of patients in a university based outpatient specialty practice and in their research projects. Job functions pertain to Lung Cancer Screening, Lung Nodule, and Interventional Pulmonology programs. The job involves working with patients, physicians and support staff to meet patient goals successfully. The primary responsibility of a patient navigator is to maintain a high degree of excellence in clinical outcomes while providing effective coordination of patient care.
ESSENTIAL FUNCTIONS:
- Manage the process for newly referred patients by developing and maintaining positive relationships with referring physicians.

Triage new and established patients.
- Provide and assist with patient care which reflects a family centered approach ultimately resulting in improved care and patient/family education.

Schedule and navigate authorizations/pre-certifications for patients receiving bronchoscopy and/or radiology services
- Ensure effective utilization of bronchoscopy block schedules.
- Develop and maintain effective physician/patient communication.
- Facilitate timely communication between departmental and referring physicians

Provide clinical and administrative support to all members of the Lung Cancer team
- Assist patients with pre and post procedure questions to assure patient compliance and well being.

Interact with co-workers, visitors, and other staff consistent with the core values of the University.
OTHER FUNCTIONS AND COMPETENCIES in addition to Essential Functions:

Coordinate the care of patients as directed by a physician.

Coordinate patient care for new patients by working closely with referring physician offices in order to provide complete medical records and radiologic images for physician review.

Determine, obtain, and organize pertinent records for new patient office visits, follow-up office visits, and procedures

Coordinate all follow up care for established patients in accordance with practice guidelines and office policy.

Assist in the precertification process for all patient services.

Assure accuracy and integrity of patient chart.

Triage referring physician telephone calls and assure timely and efficient response.

Coordinate patient appointments for office visits and procedures as directed by physician.

Provide courteous and thorough patient education.

All other duties as assigned by supervisor.

Follow all office policies regarding wait lists and voice messaging.
