Responsibilities:
The Manager – Care Management Operations will lead a regional team of Care Management employees. The Manager – Care Management Operations will provide daily supervision and operational oversight for day-to-day activities of Care Management employees with accountability to the established KPIs of the team.

Monitor and evaluate individuals’ and team performance against defined targets through utilization

Coordinate patient support with providers and care teams

Coordinate activities related to reporting and analyzing program data

Conduct regular 1:1s, performance reviews, and other feedback and coaching sessions to drive continuous improvement and growth

Coach resources with alignment to the HarmonyCares Performance Management process to ensure highest levels of efficiency, business outcomes, and patient service is continually offered within the team

Monitor daily caseloads to planned staffing levels and schedules

Provide monitoring of service levels and other KPIs, making adjustments to team allocation as necessary

Provide a point of escalation to address both patient and care team concerns

Support development of training and onboarding materials for new team members

Identify areas of quality and execution gaps for all team members and ensure appropriate remediation through coaching and feedback

Partner closely with training team to develop and maintain job resources and training aids that are reflective of current processes and workflows

Develop standard reports and collect productivity data to understand trends, speak to team performance, and identify improvement opportunities

Support development of new reports to further enhance business outcomes and team growth

Partner with leaders across the Care Management and Medical Group teams to ensure goals and targets are met

Identify process inefficiencies, bottlenecks, and areas for improvement within the team and larger organization

Develop and implement processes and enhancements to current processes to improve patient experience, operational efficiencies, and business outcomes

Qualifications:
Required Knowledge, Skills and Experience
-
Bachelor's Degree or 5 years of experience in Healthcare Administration, Public Health, Social Work, Business Administration, or another related field

General working knowledge of medical terminology and healthcare landscape
-
Excellent written and verbal communication skills, ability to act independently with minimum supervision, attention to detail, and ability to meet deadlines

Must be able to qualify and maintain HIPAA requirements

Proficiency in Microsoft Office (Word, Excel and Outlook) and data entry skills

General knowledge of insurance benefits structures

Strong working knowledge of medical terminology and healthcare landscape

Ability to support and interact with colleagues providing supervision

Strong organizational and multi-tasking skills needed

Excellent problem solving and critical thinking
