Reporting Relationship: Assistant Director, Community & Preventive Services
Starting Salary: $18.00 hourly
General Summary of Duties: This is a full-time, paid position with the Institute for Human Services, Inc. (IHS), a nonprofit organization located in Bath, N.Y. This position is contracted to work with the Steuben County Office for the Aging. The Caregiver Advocate will provide assessment, care planning, case management, support, and one on one counseling to caregivers of older persons or persons with disabilities. Collaborate effort with NY Connects to provide options counseling, information, and referrals on a wide variety of services. Provide community outreach and education regarding issues facing older and disabled Steuben County residents; facilitate support groups and assist with planning and hosting community events.

· Effective written and oral communication skills combined with the ability to problem solve.
· Strong interpersonal skills to interact effectively by phone, computer, in-person and at public events.
· Ability to work independently and as part of a team.
· Computer skills including competency with Microsoft Office.
· A valid driver's license which meets insurance standards required.
· Provide scope of Caregiver Services throughout Steuben County. This includes travel throughout Steuben County to meet with clients at Office for the Aging locations in Bath, Corning, Hornell offices and at other locations, such as home visits as needed.
· Active listening, administer emotional assessment tools, develop care plans, provide case management, and conduct reassessments.
· Maintain a strict level of confidentiality and nonjudgmental approach with clients.
· Cross-train with NY Connects to provide information, referrals, advocacy, and supported decision-making on long term services and support available in Steuben County, both privately and publicly funded.
· Offer individual counseling and family meetings to promote engagement, coping and problem-solving strategies.
· Facilitate support groups and conduct community education seminars according to trending client needs and program requirements.
· Perform timely documentation. Adhere to data entry and reporting requirements per program standards.
· Participate in public events to provide community outreach to targeted populations.
· Develop and maintain collaborative relationships with community agencies to improve access and utilization of caregiver services.
· Participate in ongoing educational and training opportunities.
· Other duties may be assigned.
Qualifications: associate degree in Gerontology, Human Services or related field and two years of related experience is preferred. A comparable combination of education and experience may be considered.
Desired Skills: Regularly required to stand, walk and stoop, kneel, crouch, drive. Must frequently lift and/or move up to 10 pounds and occasionally lift, or move up to 25 pounds. The work environment is a typical office environment with a normally quiet to moderate noise level with open access to the public. Home visits are performed on an as needed basis. The safety and cleanliness of home environments may vary.
