The Care Coordinator maintains the primary responsibility for the screening, hiring, and supervision of caregivers employed by Safe and Sound Home Care. To that end, responsibilities include but are not limited to:
· Using the company’s proven screening and hiring process, solicit, review, and assess caregiver job applicants
· Maintain accurate, timely job listings, participate in local job fairs, and identify avenues for the recruitment of quality caregivers
· Maintain caregiver employee files, including managing updates to background checks, skin TB tests and other documentation as needed.
· Conduct employee reviews at 90-day and annual intervals, as well as conducting performance improvement plans when necessary.
· Ensuring all applicants are contacted within 24 hours of receiving applications and that the company’s systems are updated appropriately
· Follow company processes to ensure orientations and other trainings are completed and reported to payroll in a timely manner
· Maintain sufficient and up-to-date resources for training materials needed to conduct new employee orientation and on-going training for caregivers. Trainings are to include company policies and procedures, as well as topics regarding the safe delivery of personal care assistance.
· Support the company’s caregiver recognition program.
· Provide technical assistance to caregivers regarding directions to client locations, care plans, client personalities and other information relevant to ensuring a successful assignment.
· Conduct periodic unannounced visits to client homes for the purpose of assessing caregiver quality and client satisfaction.
· Coordinate scheduling and assignments with the Scheduling Supervisor and provide back up to Scheduling when needed. Check the schedules daily to ensure that there is no overlap or gaps in scheduling.
· Possess a keen understanding of client needs and preferences and make caregiver assignments based on those needs.
· Participate in a rotating on-call basis to assist with client and employee needs for nights and weekends, and fill in when needed
· Maintain a pleasant and professional demeanor at all times.

Pay: $16.00 - $20.00 per hour
