Essential Functions:
- Assess, plan, implement, and evaluate total care for residents.
- Take and record residents' vital signs.
- Dress wounds; give enemas, douches, alcohol rubs, and massages.
- Apply compresses, ice bags, and hot water bottles.
- Observe residents and report adverse reactions to medication or treatment to medical personnel in charge.
- Administer specified medication, orally or by subcutaneous or intermuscular injection, intravenously, and note time and amount on resident chart.
- Make rounds with the provider and assist as needed.
- Assemble and use equipment such as catheters, tracheotomy tubes, and oxygen suppliers.
- Collect samples, such as urine, blood, and sputum, from residents for testing and perform routine laboratory tests on samples.
- Sterilize equipment and supplies.
- Prepare or examine food trays for prescribed diet and feed residents.
- Record food and fluid intake and output.
- Bathe, dress, and assist residents in walking and turning.
- Clean rooms, make beds, and answer residents’ calls.
- Wash and dress the bodies of deceased persons.
- Inventories and requisitions supplies.
- Audit medication cart.
- Order and Re-order medication.
- Liaison with Medical director, external partners, and families.
- Assist with admission and discharge.
- All other duties as assigned.
Supportive Functions:
- Assists in the orientation of new personnel.
- Recognize your role as part of the QAPI efforts of your organization.
- Attend trainings to build understanding and capacity to undertake QAPI work.
- Carry out QAPI roles and responsibilities as assigned.
- Follow established policies and procedures in support of QAPI efforts.
- Look for and share with leadership ideas for improvement in the organization.
- Communicate to leadership upon witnessing a positive outcome of a QAPI project or detecting barriers preventing project success.
- Support QAPI efforts both verbally and non-verbally, including adjusting performance and practice in accordance with QAPI initiatives and findings.
Other Responsibilities:
- Regular attendance in conformance with standards is essential.
- Due to the cyclical nature of the healthcare industry, employees may be required to work varying schedules.
- Compliance with all applicable rules and regulations for the safe and effective operation of the facility.
- Grooming: All employees must maintain a neat, clean, and well-groomed appearance.
- Physical Requirements: These are physical and mental requirements of the position as it is typically performed.
- Inability to meet one or more of these physical or mental requirements will not automatically disqualify a candidate or employee.
Qualifications/Basic Job Requirements:
For LPN Position:
- Graduated from an accredited LPN program.
- Currently licensed/registered in applicable State.
- 6 months to 1-year skilled nursing experience preferred.
- CPR certification upon hire and must remain current during employment.
- Fingerprint Clearance Card upon hire and must remain current during employment.
- IV Certification or willingness to attend training provided by the facility.
For RN Position:
- Associate of Sciences (AS) degree in Nursing or a Bachelor’s Degree (B.S.) in Nursing from a four-year college or university.
- 6 months to 1-year skilled nursing experience and/or training preferred.
- Current RN license in the State of Arizona and remain in good standing throughout employment.
- CPR certification upon hire and must remain current and in good standing throughout employment.
