RESPONSIBILITIES for Licensed Vocational Nurse (LVN):
Provides skilled nursing care to patients in their own home under the supervision of a Registered Nurse. Provides service in accordance with Hope Hospice’s philosophy, standards, and policies.
REPORTS TO: Director of Patient Care Services
FUNCTIONS for Licensed Vocational Nurse (LVN):
- Perform home visits under the direction of the Nurse Case Manager and/or supervisor/designee, including the following: providing wound care, pain and symptom management follow up, Mediset refills, unscheduled (crisis) visits, patient and family teaching.
- Provide services under Continuous Care for Hope Hospice patients working with Nurse Case Manager, supervisor and designee.
- Assist with Intake Coordination and other special assignments requested by her supervisor or designee.
- Provide palliative nursing care and ongoing support in the home as ordered on the plan of treatment and nursing care plans.
- Function as a team member demonstrating collaboration with interdisciplinary team.
- Adequately assess and reassess pain. Must utilize appropriate pain management techniques. Educate the patient and family regarding pain management.
- Carry out the pain management and symptom management plan to keep the patient as comfortable as possible. Reassess pain and notify the registered nurse when pain is not manageable on the current medications.
- Respond to the physical, mental, emotional, and social needs of terminally ill patients and their families.
- Perform all aspects of patient care in an environment optimizing patient safety.
- Documentation is entered into the Matrixcare Electronic Medical Record System. Documentation is completed within agency guidelines and to meet regulatory requirements.
QUALIFICATIONS for Licensed Vocational Nurse (LVN):
- Possess a current license to practice as a Licensed Vocational Nurse in the State of California
- Experience as a licensed nurse with preferably 1 year in either an acute care setting or long-term care setting.
- Prior hospice or home care experience is preferred.
- Demonstrated ability to assess and respond to the needs of patients and families in varied settings
- Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
- Possess a valid California driver’s license, valid auto insurance, and an automobile for field use
- Current CPR Certification.
- Requires manual dexterity sufficient to provide hands-on care to the patient under variable conditions and settings
Job Type: Part-time
Pay: $40.00 - $43.00 per hour
