The Community Patient Navigator is a part-time position with Health Promotion Council of Southeastern Pennsylvania, Inc.’s, (HPC) Family Services Department responsible for providing care coordination services for the Navigating, Educating, and Supporting Together (NEST) program. HPC’s NEST project works to eliminate health care disparities by providing targeted, patient-centered outreach, marketing, education, care coordination and navigation services that focus specifically on medically underserved Latina women including, uninsured and/or low-income, migrant Latina women residing in the Philadelphia and Norristown areas of Pennsylvania.
The Community Patient Navigator works in collaboration with the NEST team to implement community outreach and education and recruits' participants in need of navigation services to increase access to and coordination of breast and cervical health services for medically underserved Latina women. The Navigator works closely with the Lead Community Patient Navigator and Assistant Director for Family Services to ensure that project deliverables and timelines are met. Additionally, the Navigator interfaces regularly with community partners, and community residents. More specifically, responsibilities include: providing community education sessions on women’s health while mitigating social determinants of health barriers. The Navigator facilitates referrals as needed; coordinates care for women needing a clinical breast exam and/or mammogram and/or further diagnostic or treatment services; assists with collection of data and reporting; and assists with coordinating meetings.
The Navigator will provide individualized health education, promote health and medical care literacy and provide medical system navigation services for individuals and families in the Southeastern PA region. The Navigator maintains contact with clients via in-person visits, email, phone and videoconferencing. Through referrals from health systems and community organizations, the Navigator assesses needs, coordinates care at home and in the clinical settings, and supports other needs within the home. The Navigator collaborates with health care, social service, and other agencies to function as an advocate and liaison between families, promoting healthy communication. This position will have contact with enrolled families and the corresponding medical and social systems. This position will require travel throughout Southeastern Pennsylvania, therefore a driver’s license and access to a reliable vehicle with current registration, and current auto insurance is required. Some evening and weekend work will be required.
Responsibilities:
- Builds and maintains strong relationships with organizations.
- Networks and schedules community presentations for education and recruitment purposes.
- Provides care coordination services to women entering the program.
- Community Education and Patient Navigation
- Conduct outreach in community settings to recruit participants for group-based and one-on-one education, and patient navigation services.
- Coordinate all aspects of community outreach and educational workshops, including identifying sites, coordinating logistics, conducting education, and completing appropriate reports.
- Complete intake services and assess social determinants of health that impact the woman’s ability to access and receive health care.
- Provide one on one education for eligible patients along with navigation and case management services.
- Conduct follow-up with workshop attendees that are interested in accessing a Clinical Breast Exam and/or mammogram or cervical exam/cancer screening.
- Schedule/coordinate patient appointments, including facilitating connection between patient and current primary care provider or linking patient to a new primary care provider; facilitating transportation to appointment, if needed; and accompanying patient to appointment, if needed.
- Provide interpretation services during appointments, if needed.
- Conduct appointment follow-up, if needed, including coordinating/scheduling follow-up services, and providing additional referrals for supportive services.
- Documents all client contacts using required written forms: assessment forms, progress notes, referral forms, discharge and transition plans.
Skills:
- Fluent in Spanish and possessing a profound understanding of Latina culture.
- Excellent understanding of the health system and community-based organizations in Philadelphia and Norristown.
- Experience successfully managing competing timelines.
- Ability to establish priorities, and work both independently and in a team environment to meet objectives with minimal supervision.

Experience:
- At least two years' experience working with diverse populations, low-income individuals, individuals from different religious backgrounds.
- At least two years' experience conducting community health education programs.
Education:
Bachelor's degree in social work, public health, or a related field preferred
