Position Summary:
The Patient Services Specialist represents Roots Community Health Center, working as part of a team in a highly visible setting. This position provides superior client/patient service, and interacts with team members/clinic personnel, employees of other departments, physicians’ offices and hospitals, as well as the public. This position provides outreach and services for low-income and/or high-risk individuals such as formerly incarcerated, homeless and substance using populations potentially eligible for Medi-Cal funded services. This position provides outreach and peer mentoring to increase access to the Medi-Cal Program, facilitation of the Medi-Cal application and linkage to Medi-Cal services.
Duties and Responsibilities:

Provide Medi-Cal outreach activities at selected community sites to low-income high-risk individuals and families, providing information about services and benefits that the Medi-Cal program offers, assessing Medi-Cal eligibility, enrolling individuals into Medi-Cal, linking clients with health and mental health service providers.

For individuals and families who are known to be Medi-Cal beneficiaries, make referrals to connect them to appropriate Medi-Cal covered health services.

Attend program meetings to discuss status of referrals, challenges/barriers with the following up with patients, and/or outside specialist. Report to the program administrators the current total of referrals for each program.

Attend and participate in MAA/TCM Implementation Trainings.

Attend organizational and other trainings and meeting related to job role.
Competencies:

Experience working in a non-profit organization, or a community clinic preferred.

Solid organizational skills including attention to detail and multi-tasking.

Strong working knowledge of Microsoft Office.

Ability to work with people from diverse backgrounds.

Strong interpersonal communication skills.

Willingness to work a flexible schedule both remote and on-site.
