GENERAL REQUIREMENTS
Under the general supervision of the Director of Residential Services, with general responsibilities of residential management, including the day to day operation of residential habilitation units and supervisory oversight of the care of residents of such units.
Administration
FUNCTIONAL RESPONSIBILITIES
- To function as the on-site supervisor of four supportive living homes relative to the day-to-day operational management.
- To function as a member of the Residential Program Management Team.
- To assist the Financial Specialist with the physically inventory of each home specifically identifying agency and client owned property at least on an annual basis.
- To carry a cell phone for on-call issues for their respective supervised home.
- To conduct on site unannounced visits to respective supervised home on each shift 1time per month.
- To follow and implement position responsibilities as denoted in the agency's Supervision Plan.
Personnel
- To work with the Team Leader for scheduling the respective home in order to assure coverage as noted on the staffing plan and to minimize overtime accumulation of staff.
- To train new staff in emergency and safety policies, practices of the Supportive Living program, specific operational duties of the home, individual specific training per each client' s ISP and individual characteristics. Train each staff on the TSNI.
- Ensure there is a med trained staff to dispense medications at the correct time.
- MAR reviews weekly including following up with any noted med variances.
- Ensure all staff is trained on the proper shift change.
- With the Team Leader, administer on-going evaluations of staff performance and to conduct probationary evaluation for new staff as well as the annual performance evaluation of the respective staff of each home.
- To complete all personnel documentation as necessary including corrective action reports, training documents and other documentation as specified by the Residential Director and the Chief Operating Officer.
- To complete grocery shopping every two weeks. Ensure there is always 72 hours' worth of meals/snacks in the home. Ensure fresh fruits/vegetables are available. Ensure that the assigned budget for groceries is kept.
- To complete supply shopping monthly to ensure house hold supplies are sufficient.
- Complete shopping for service recipient hygiene items monthly.
- Ensure all staff is completing daily documentation correctly.
- Ensure staff is completing daily shift responsibilities.
- Ensure you have timely completed/followed up with any Reportable Incidents.
- Keep Team Leader up to date on issues/concern going on within the Supportive Living
Consumer Services
- To assist in the training of staff relative to personal assistance needs of clients, e.g. (assistance with eating, dressing, toileting, bathing, dressing, medication administration and other as needed and/or required).
- To communicate on a scheduled basis or as needed, with consumer families and/or conservators, support coordinators relative to consumer's compatibility to staff and residence, and to coordinate any visits with or by families.
- To coordinate with the Director of Health Services with regard to any health or medical issue regarding
- Ensure the service recipient has a scheduled staff to take them to all doctor's appointment.
- Must attend all Neurology, Mental Health and surgical appointments.
Support Plan
- To participate in the Client’s Circle of Support Team as the Lead Staff Person in the development of the individual support plan for the client as a part of the client's overall service plan.
- To be responsible for maintenance and review of the client's individual records including the main unit file maintained in the home, daily or monthly checklists, and other documents as required, and to coordinate the transfer of records between all parties.
Financial
- To familiarize and comply with all regulations of the agency's Consumer Personal Finances Policy.
- Check the Personal Funds Log weekly to ensure its being reviewed at all shift changes and ensure the money and receipts are being handled correctly.
- To maintain a system and practice of preserving on a monthly basis all receipts of purchase as required of individual consumer personal purchases not made by the purchasing coordinator.
Operations
- To develop an operations system for each home relative to specific household duties e.g. (Food Procurement and Storage, Menu Development and Meal Preparation, Clothing Designation and Laundry, Procurement and Storage of Household Supplies, Housekeeping and Maintenance, Personal Care and Bathing of Consumers) and the development of a schedule and staff assignment of each operation responsibility.
- To schedule with agency maintenance staff, the regular as well as any necessary maintenance and/or repair according to maintenance request procedures.
Emergency Procedures
- To assist in the development of specific emergency procedures for each respective home. Specific procedures are detailed in each home's procedural manual.
- To notify proper emergency professionals as well as the administrative staff as designated in the procedural manual in the event of an emergency situation.
- To assure that a monthly emergency evacuation drill is conducted and documented, and that all staff participate on a rotational basis.
- To take on-call and management responsibilities in the absence of any Team Leader.
All other duties as assigned
QUALIFICATIONS
- Must have a minimum of a high school diploma/GED, some college preferred
- 2 years' experience in working with persons who have disabilities is preferred
- Management experience desired
