Job description
We are looking for a competent Home Health Care Coordinator to assist the Agency Administrator in oversee the growing operations of the company. The ideal candidate will be a hard-working professional able to undertake a variety of tasks. This individual must be analytical, technology-savvy, able to work well under pressure and able to work with little or no supervision. This person will be comfortable working with a high degree of attention to detail and discretion as well as incorporating new and effective ways to achieve better results.
Responsibilities
As Home Health Care Coordinator, your responsibilities shall be as follows:
Client Engagement Responsibilities
- Receive and follow up on referrals for new clients received from various referral sources.
- Maintain accurate and up-to-date electronic medical records of potential, active and inactive clients, in line with industry best practices and agencies policies and procedures.
- Ensure documentation and implementation of individual client’s emergency preparedness plan.
- Organize and execute periodic supervisory visits and engagements with clients.
- Identify opportunities for cross-selling of other goods and services to current clients.
- Engage with clients for the development and optimization of care schedules to meet client’s requirements and match agency capabilities.
- Maintain an accurate list of available or unstaffed shifts for each client.
Staffing / Recruitment Responsibilities
- ·Identify, recruit and on-board aides and nurses to be staffed on cases in line with agency policies and procedures.
- Use available aides and nurses’ databases to identify aides who may be able to cover available shifts.
- Work with administrator to ensure that caregiver credentials are up to date, preparing monthly and quarterly reports showing status of caregiver credentials.
- Perform weekly review of caregiver visit notes, tasks performance and caregiver electronic visit logs.
- Implement caregiver performance appraisals and disciplinary actions in line with policies and procedures.
Other Business / Administrative Responsibilities
- Assist in developing, maintaining and implementing agency emergency preparedness plans.
- Assist in developing, maintaining and implementing up to date agency policies and procedures.
- Oversee the quality assurance and performance improvements processes of the agency.
- Assist the agency clinical director with the set up and organization of client assessments in line with agency policy.
- Assist with filings, customer billings, budgets and financial planning of the agency and other administrative tasks as required by the agency.
Skills
- Proficiency in using a cloud-based Home Care management solution such as Generations, ClearCare/Wellsky, AxxessHome Care etc.
- Excellent ability to communicate verbally, orally and consistently. Candidate must be great listener and empathetic
- Proven experience in customer service, customer support, business development, office admin duties, sales and marketing, business development or in another relevant administrative role
- Experience with Medicaid Waiver programs in Maryland, DC or Virginia preferred.
- Working knowledge of office equipment, laptops, smart phones, fax machines, printers etc
- Thorough understanding of office management and record filing procedures
- Excellent organizational and time management skills
- Analytical abilities and aptitude in problem-solving
- Excellent written and verbal communication skills
- Proficiency in MS Office and other office work tools like MS Teams, Zoho, Google Workspace, MS Sharepoint etc
- Bilingual ability (Spanish) will be a plus
Job Types: Full-time, Contract
Pay: $52,000.00 - $57,200.00 per year
