What You Will Do:
As the Social Worker, you will help patients and their families to enhance or restore their capacity of optimal psychological, emotional, spiritual, social, and physical health. Identify coping skills and strengths utilized by the patient and family. The social workers function as an integral part of the interdisciplinary team by addressing the psychological stressors and collaborating with the team to enhance the quality of end of life care for the patient and their family.
Qualifications:
- Complete an initial psychological evaluation within the limits set by the conditions of participation and/or state regulations.
- Demonstrate a working knowledge of the conditions of participation and state regulations governing hospice care.
- As allowed by the patient and family, assist with advanced care planning to ensure that all treatments meet the wishes of the patient receving care. Advanced care planning can include advanced directivs, DNR's, DPOA's, financial planning and funeral planning.
- Assist the patient and family to understand, accept, and follow medical recommendations and provide services planned to assist the patient and family in achieving the optimum social and health adjustment within their capacity.
- Utilize available resources to assist the patient and family to live better within the limitations of the illness.
- Records and reports observed social and emotional changes within a timely manner.
- Educate, mediate, and problem solve with patient and families regarding ethical dilemmas that may arise.
- Demonstrate the ability to prioritze and balance daily schedules and completes duities in appropriate time frames.
