Job Overview:
Calvert Internal Medicine Group is seeking a dedicated Care Management Supervisor to join our healthcare team. This position is an integral member of the population health team.
Essential Functions:
- Conducts comprehensive assessments of patients, including their medical history, medication, treatment plans, and psychosocial needs.
- Collaborates closely with healthcare providers, patients, and families to develop and implement individualized care plans.
- Monitors and evaluates patients’ progress, adjusting care plans as needed.
- Serves as the main point of contact between the patient and healthcare providers, ensuring seamless communication and care coordination.
- Instructs and educates patients on healthcare provider instructions.
- Links patients to social service programs and entitlements.
- Maintains up-to-date records and care notes.
Supervisory Responsibilities:
- Develops, implements, and manages the care management activities aimed at improving population health outcomes and maximizing value-based care initiatives.
- Champions innovation in clinical practice.
- Develops pilot projects of evidence-based ideas.
- Creates management systems that promote standardization of workflows.
- Utilizes data to identify population health program needs.
- Initiates program interventions targeting new patient populations.
- Oversees and manages programs and projects.
- Supervises, directs, and coordinates care management activities.
- Develops and leads care management team.
Knowledge, Skills, & Abilities:
The care manager supervisor possess -
- A passion and commitment to excellence.
- Effective interpersonal, verbal, and written communication and presentation skills.
- A knowledge and proficiency in the use of metrics and measures in managing programs and services.
- Strong organizational skills to manage multiple projects, issues, staff and priorities.
- Knowledge of managed care principles, concepts, and standards of practice for care managers.
- An ability to mentor and coach associates to accomplish goals, provide objective evaluation of associates’ performance, and implement strategies to improve individual and team-based performance.
Additionally, the care manager supervisor will -
- Offers strategic leadership and guidance.
- Develops, implements, and assuring the completion of short and long-term goals and objectives.
- Analyzes, evaluates, and recommends operational efficiencies, process design and organization structure.
- Utilizes data to target opportunities for improvement.
- Designs workflows to meet program goals and metrics.
Qualifications
- BSN or MSN required.
- State of Maryland RN license required.
- Certified Care Manager (CMM) required.
- 7 – 10 years progressive care management experience.
- 5 years of management experience to include considerable experience in care management, population health, leadership, coaching, and mentoring.
