Qualifications:

· A minimum of 1 year of experience working with older adult population
Job Goal:

Hours:
· Monday through Friday, 9:00am to 5:00pm
Reports to:
· Director of Resident Services
Duties:
· Provide information and referrals to residents and families on programs and resources at local, state, and federal levels
· Interview and screen residents for eligibility for programs and entitlements and assist with the application process
· Make apartment visits to assess needs
· Assess the needs of residents, including physical, mental, social, and financial needs and develop a plan for service delivery
· Monitor and evaluate service delivery and reassess as necessary
· Establish links with agencies and service providers
· Serve as property Services for Independent Living (SIL) Coordinator
· Contact family members as needed and allowed, to obtain information, discuss service arrangements, or to encourage family involvement
· Plan and implement monthly programs and activities to meet the needs of residents, including establishment of social, cultural, educational, and recreational programs
· Encourage residents to obtain Power of Attorney, Living Will, and Will documents
· Attend seminars, workshops, conferences, etc. on topics relating to job description
· Develop volunteer programs and informal support networks
· Conduct New Resident Orientations in a timely fashion
· Manage case files of residents
· Make presentations on selected topics of interest
· Work in accordance with the Social Work Code of Ethics
· Work alongside Management and other departments within the organization
· Performs other related duties as assigned
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
Expected hours: 40 per week
