Job Summary:
This position reports to multiple Rennes facilities within NE WI (De Pere, Appleton, Peshtigo).
The Lead Registered Dietitian Nutritionist (RDN) provides nutrition services including medical nutrition therapy (MNT) utilizing the nutrition care process; assessment, diagnosis, intervention, monitoring and evaluation for short-term and long-term residents primarily in a Skilled Nursing Facility setting. Occasionally serves as resource for Residential Care and Apartment Complex (RCAC) facilities and residents. Collaborates with the interdisciplinary team to ensure resident needs and preferences are being met through high quality nutritional meals and service.
Duties/Responsibilities:
- Provides feedback and advises the facility administration and dietary manager on all clinical aspects of the dietary department; collaborates with Senior Clinical Dietitian and Corporate Food Service Director as appropriate
- Assesses the nutritional status of residents inclusive of lab value interpretation, consumption patterns, weight maintenance, food/medication interactions, dietary compliance, resident independence, and overall resident well-being
- Completes all nutrition related assessments and follow-ups regarding hydration status, admission, quarterly, significant change
- Codes and completes sections of the minimum data set (MDS) that pertain to nutrition
- Ensures appropriate documentation of nutritional assessment and recommended intervention in the resident chart and/or care plan; reviews the documentation of others regarding nutritional concerns and responds appropriately
- Participates in consultations with residents and families to evaluate nutritional needs and in turn, develops, maintains, and updates resident care plans as appropriate
- Participates as an active member of the facility interdisciplinary team
- Attends care conferences for residents at high risk for nutrition related concerns
- Reviews proposed menu changes to ensure nutritional content adequacy
- Supervises the development of specialized diets using appropriate dietary modifications where necessary
- Completes reports in compliance with Federal, State and local guidelines; stays current on all guidelines and regulations
- Ensures compliance with nutritional related areas with regard to CMS and WI DHS regulations; participates in survey and regulatory processes
- Assists with development and updating of facility policy and procedures
- Performs other tasks as assigned

Education and Experience:
- Bachelor’s degree in the field of dietetics with courses in food preparation and service
- Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR)
- At least one year of experience in a related field of dietary work, preferably in long term care is preferred
