About the Role:
The person in this role will assess, plan and advocate for the patients’ health needs. As a trained health professional, you will monitor and coordinate patients’ treatment plans, guide them to understand the status of their health, and check in on their progress. The patient care coordinator also collaborates with other members of the care team to look after patients within the healthcare facility.
Essential Job Functions:
· Coordinate patient records within the Electronic Medical Record
· Retrieve important patient data in EMR.
· Ensure complete and accurate patient registration, which includes recording patient’s insurance.
· Apply knowledge of funding resources, services, and clinical standards to everyday tasks.
· Discuss and consult health history, diagnosis, and needs with patients and family members.
· Develop care plans with care team to ensure patients’ safety.
· Educate patients and family members about health conditions, medications, and instructions.
· Follow facility guidelines, healthcare laws, and regulations.
· Contact insurance companies to ensure approval requirements are met.
· Provide medical information to financial services for maximum health facility and patient reimbursements.
· Establish and maintain relationships with service providers.
· Maintain professionalism and treat patients with respect.
· Ensure adherence to quality standards and procedures in all aspects of Patient care coordinator.
· Conduct regular quality inspections to identify defects and deviations from established standards.
· Collaborate with cross-functional teams to implement quality improvement initiatives and drive continuous improvement.
· Analyze quality data to identify trends, root causes of issues, and areas for improvement.
· Participate in quality audits and assist in the development of corrective action plans.

Education/Experience (Minimum Requirements):
- High school diploma or GED certificate.
- Knowledge of ECW, EMR, and medical terminology, including CPT codes, ICD-10 codes, patient registration, charge entry functions or other billing related experience is preferred.
- Computer proficiency.
- Minimum of one year of experience in a medical office setting.
- Scheduling software experience preferred.
- Knowledge of common medical terminology.
- Prior patient records experience, ideally in a medical clinic environment.
- Excellent communication skills and phone etiquette.

Medical Clearance:
You must complete Comprehensive Primary Care’s medical clearance requirements, which include, but may not be limited to, evidence of Hepatitis B, Pneumococcal screen, and a Tuberculosis screen, or have an approved medical or religious accommodation that precludes you from being vaccinated against these diseases.
