Absolute Hospice is hiring a 7 ON/ 7 OFF On-Call Registered Nurse to provide end of life care to patients wherever they call home.
Responsibilities:
- Assume primary responsibility for the hospice patient and their family that includes assessing, planning, implementing, and evaluating care needs during on-call hours
- Completing initial evaluation and re-evaluating the patient’s nursing needs during each visit.
- Provide appropriate support at the time of death and perform bereavement assessment if requested.
- Report On-Call activities to the agency leadership and Case Managers at the end of each call shift
- Maintain and complete appropriate care coordination notes for visit notes, and other pertinent clinical information in a timely and accurate manner
- Assist, provide, or arrange for delivery of medications and supplies needed after business hours, weekends, and/or holidays
- Under the physician’s order, admits patients eligible for hospice services in collaboration with the triage team
- Reports patient status and need for other disciplines to agency leadership, attending physician and the hospice physician
- Develops patient care plan that specifically addresses identified patient problems; nursing problems and goals that has been identified as mutually agreed upon goals with patient(s) during on-call hours, weekends and/or holiday(s)
- Provide positive communication to physicians, patients, families, facilities, visitors, and other organization personnel.
Requirements
- Graduate of an approved nursing program/school is required. Bachelor's Degree, preferred.
- At least 1 year of Hospice experience is required
- Active Michigan RN License is required.
- Active AHA BLS Certification is required.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.
- Ability to work in a field setting and exhibited ability to make sound nursing judgments; must be able to work independently
- Ability to assess patient needs and formulate individualized patient care plans to meet those needs
- Knowledge of physical, psychosocial and spiritual needs of terminally ill patients and their caregivers
- Effective written and verbal communication skills
- Current driver’s license and automobile in good working condition with proof of auto insurance

- On 7 days and then off the next 7 days
- 5:00 pm – 8:00 am, Monday through Thursday & 5:00 pm Friday - 8:00 am Monday
