We are seeking a dedicated and compassionate individual to join our team as a Housing Specialist. Project Transitions is dedicated to serving people with HIV and AIDS by providing housing, comprehensive support services, recuperative care, and hospice in compassionate and caring environments.
As a non-profit organization focused on aiding those in need, we are committed to helping individuals living with HIV transition from homelessness to stable, independent living. The Housing Specialist will play a crucial role in this process by providing comprehensive case management services to clients, helping them navigate the complexities of securing stable housing and accessing necessary support services.
In this role, you will work closely with our clients to assess their needs, develop individualized case management plans, and connect them with appropriate resources and support networks. You will support clients in obtaining and maintaining stable housing, while also assisting them in accessing healthcare services, employment and educational opportunities, and other essential resources. Your empathy, understanding, and advocacy will be key in building rapport and trust with clients, empowering them to achieve their goals and overcome challenges.
Responsibilities
· Conduct client assessments to determine housing and support needs
· Develop individualized case management plans in collaboration with clients
· Assist clients in navigating resources to address housing barriers
· Support clients in being accountable for their housing and personal goals
· Maintain completed documentation for client files
· Support Team lead and Director of Client Services (DCS) in annual audits
· Assist clients in securing and maintaining stable housing
· Keep accurate and timely case notes for all clients on caseload
· Connect clients with community resources and support networks
· Advocate for clients and ensure their needs are met
· Assist with program eligibility documentation and lease files
· Provide crisis intervention and emergency support as needed
· Collaborate with Team Lead and DCS to coordinate client care
· Work with partner agencies and be an integral part of client care team
· Support Property Manager in conducting the CoA and TDHCA recertification
· Perform other duties as may be assigned by DCS
Requirements
· Bachelor's degree in social work, psychology, or related field
· Minimum of 2 years of experience in case management or related field
· Must be bilingual English-Spanish speaker
· Knowledge of homelessness and housing support services
· Familiarity with community resources and social service programs
· Strong interpersonal and communication skills
· Ability to work effectively with diverse populations
· Strong organizational skills and attention to detail
· Valid driver's license and reliable transportation
Job Type: Full-time
Pay: $20.81 - $25.06 per hour
