Residential House Supervisor will provide an array of services which are necessary to support persons with developmental disabilities to live in the community. This position provides on-site supervision to the assigned home, including maintenance of the home, as well as support for the individuals living in the home.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Demonstrates and performs as an effective adult role model for the people that we support.
- Adheres to assigned schedule
- Coordinates and transports individuals to medical and dental appointments
- Ensures staff is aware of medication changes/treatment changes
- Responsible for monthly staffing schedules and ensures the homes stays within allocated hours
- Monitors staff documentation: Activity Notes, ISP Tracking, Fire/disaster drills, Behavioral Support Plans, and House Logs and ensures that all necessary documents are accurate and turned in monthly to Residential Coordinator
- Ensures that each individuals medication is received monthly and reviews Medication Administration Reports for assigned homes
- Ensures and monitors homes for cleanliness, furniture is in good repair, adequate food supply and ensures that ISPs are current for each individual
- Ensures medication refills, medication report completed after each medical visit as well as PRN medications
- Ensures staff and coordinators are aware of medication changes/treatment changes
- Communicates professionally and treat the people we serve, parents, service monitors/coordinators, and InCommunity employees with respect
- Ensures that CLAs are in compliance with Healthcare Facilities Regulations and any other governing bodies
- Monitors house funds, i.e. making funds available for recreational events, document individual spending of funds, collect receipts
- Notifies coordinator immediately of any incidents
- If appropriate, complete Critical Incident Reports and submit to coordinator within one day.
- Notifies supervisor of problem areas needing counseling or disciplinary action for subordinate staff/document same
- Keeps abreast of training opportunities
- Adheres to InCommunity policies concerning confidentiality of consumer information
- Adheres to InCommunity documentation standards for participant and provider records
- Ensures that all mail is turned into the main office in a timely manner
- Monitors the vehicles assigned to the homes i.e. cleanliness, good repair etc.
I. Training/Program Responsibilities
- Implements activities as identified in each individuals service plan.
- Engages individual in activities of choice, which reinforces ISP goals.
- Promptly reports any emergency/crisis situation to supervisor and follows up by completing appropriate incident report within twenty-four (24) hours.
- Coordinates transportation and supervision for individuals as dictated by individual program.
- Ensures that all information regarding daily activities, problems and needs of individuals are recorded properly in daily log.
- Provides training and assistance in changing appropriate apparel on a daily basis to ensure comfort and well being for individuals.
- Ensures that proper maintenance is performed on the van when necessary.
- Attends annual ISP meeting for each individual
- Coordinate individuals outings for community involvement and shopping for their personal supplies.
- Is responsible for staffing the group home, relief staff, and leave scheduling
II. Responsibilities Relating to Health and Safety
- Inspects home to ensure that home meets all safety requirements, checking for clutter and dangerous objects.
- Corrects deficiencies or submit written report to supervisor outlining deficiencies by end of shift.
- Conducts evacuation drills, both fire and severe weather; records results of each drill.
- Follow program plan for meeting all potential emergencies and disasters, such as medical, fire, severe weather and missing persons.
- Notifies supervisor of all medical emergencies as outlined by policies and procedures.
- Checks to ensure that supplies are clearly labeled.
- Reports any problem with equipment used by residents to proper individual or department
- Ensures cleanliness of home
III. Housekeeping Responsibilities
- Ensures that laundry is washed, ironed and properly put away daily.
- Ensures menus are nutritionally sound and appropriate for all individuals in the homes; ensures menus are posted weekly in accordance with regulations and previous three months menus are filed in the home
- Purchases groceries for the home on a weekly basis, in accordance with approved menus.
- Purchases needed offices supplies
- Purchases required SMS and SME.
IV. Administrative Responsibilities
- Ensures compliance in the area of responsibility during assigned hours with local, state and federal rules and regulations applying to Community Living Arrangement.
- Complies with the organization policies and procedures governing the use of behavior management programs for controlling maladaptive or problem behavior.
- Complies with policies and procedures to ensure that confidentiality requirements are upheld.
- Supports and protects the fundamental human, civil, constitutional and statutory rights of clients and families as defined by agency.
- Demonstrates competence in handling medical emergencies in accordance with InCommunity policies and procedures.
- Maintains accurate and complete records of all individuals personal spending monies in accordance with rules and regulations; completes monthly reconciliation and submits report to Manager of Residential for approval and final submission
- Maintains accurate and complete records of house spending (including credit card purchases and petty cash purchases) in accordance with rules and regulations; completes monthly reconciliation and submits reports to Manager for Residential for approval and final submission
- Reports to work promptly and on time and does not leave work until assigned shift ends or has approval from supervisor. Observes policies/procedures governing requests for annual, sick or other leave time.
- Observes all other personnel policies and works closely with supervisor to ensure compliance.
- Abides by code of ethics and serves as a good role model for clients.
V. Record Keeping Responsibilities
- Maintains records in the home to meet local, state and federal guidelines
- Makes individual related entries in daily logs that are legible, dated, authenticated by signature and positions, in ink and without use of symbols or abbreviations.
- Maintains tracking records in accordance with Individual Service Plan to comply with program requirements and expectations of supervisor.
- Records contacts with individuals family or guardian.
- Maintains current log for approved family visitations and/or outing with friends.
- Maintains medication records in accordance with policy
- Maintains records of evacuation drills
- Maintains appropriate documentation relating to health, safety and infection control.
- Maintains records of van log data sheets.
- Adhere to InCommunity policies concerning confidentiality of consumer information.
- Adhere to InCommunity documentation standards for participant and provider records.
- Other duties as assigned
EDUCATION/ EXPERIENCE
High school diploma or equivalent and one year coordination experience in the area of developmental disabilities, preferably with the focus being in community-based programs and knowledge of family systems and structures
