LEAD HOUSING CASE MANAGER

Hours: Monday - Friday 8 AM - 5 PM
Salary Range: 48,000.00 - 51,000.00
POSITION SUMMARY:
The position of the Lead case manager provides the necessary stability to the residents of the property through initial move-in orientation of community rules and program rules, case management services, and enforcement of all property rules and state and federal laws for residents residing at Native American Connections properties. This position is responsible for a small caseload in addition to supervision responsibilities and administrative tasks. The ideal candidate has a strong understanding of Permanent Supportive Housing, the Housing First model, providing case management services, and community resources. The ideal candidate also has experience using the SPDAT and HMIS.
RESPONSIBILITIES:
- A full understanding of NAC’s philosophy of housing and resident wellness
- Enter case management information in HMIS software program for case management data.
- Complete initial case plan and SPDAT within 3 days of move-in; complete intake forms with residents and evaluate the level of need for case management services; complete and update resident case management plans and files monthly.
- Administer the SPADT assessment tool in suggested intervals.
- Develop and facilitate life skills curriculum for individuals and groups.
- Connect residents to community resources that offer life skills, education, and employment services as needed.
- Maintain a strong working relationship with NAC outpatient services and refer for behavioral health counseling as appropriate.
- Provide referrals for emergency stabilization as needed; provide referrals for and/or deliver Financial Education; referrals for food boxes; manage bus pass program.
- refer (and follow-up) to DES, applications for food stamps, and AHCCCS as needed; work individually with clients to remove barriers to employability.
- Maintain timely, accurate case management written file documentation on participant attendance, services and progress, rental history, and recertification of income.
- Lead weekly staffing to update all residents’ progress.
- Maintain daily shift reports and document all resident interactions.
- Conduct monthly community meetings and work to improve the payment history, health and safety issues, and resident conduct through the community through one-on-one education of the residents as needed.
- Responsible for creating and maintaining staff schedules and monthly activity calendars.
- Responsible for completing various reports in a timely manner, including funder reports.
- Supervises case aides and serves as the property’s first-line supervisor for Services staff.
- Responsible for maintaining inventory and submitting orders for program supplies.
- Other duties as assigned.
EDUCATIONAL REQUIREMENT:
- Bachelor degree preferred
WORK EXPERIENCE/SKILLS REQUIREMENT:
- 3+ years’ experience in service delivery with increased responsibility OR any combination of related education, professional training, or work experience that demonstrates the ability to successfully perform duties.
- 1+ year supervisory experience
- Knowledge of Permanent Supportive Housing and Housing First model
- Knowledge of the economic, educational, and social problems of Native Americans and referral services
- Must be able to work well with others in a team approach.
- Excellent communication skills – written and oral
- Experience in working with the Native American population preferred.
- Possess and maintain a valid Arizona driver's license reliable transportation and clean driving record.
