Position: Social Worker- Case Coordinator
Job Description:
General Description: Under general supervision, provides case management along with service and resource coordination in a human services environment, particularly with clients that are low-income and in crisis or emergency housing situations.
Primary Job Responsibilities & Duties:
Conducts phone intake assessments and in-person appointments. Manages documentation for organizational purposes, as well as management and reporting for community support services.
Assist clients and/or their families to facilitate achieving service plan goals, developing life skills, and/or sustaining financial stability, conducts activities and monitors the service environment in a manner to maximize client success and well-being.
Attends community meetings that pertain to resources specific to Community Support services.
Other Job Duties:
- Works with community partners to maximize the assistance for clients.
- Advocates on behalf of clients and families for services, basic needs, and other related issues.
- Works directly with the ECM department for proper management and payment of housing
services and housing deposits. Help facilitate housing transition and short-term housing
solutions.
- Composes or prepares correspondence, case notes, narrative and technical reports,
notifications, and related documents using computer-based applications.
- Conducts comprehensive, client-centered social work activities in accordance with best
practices.
- Stay abreast of community resources and procedures for referring clients to appropriate
resources.
- Performs other duties and assumes other responsibilities as assigned by supervisor.
Supervises: This position will not supervise MSW & BSW Interns, as well as Volunteers.
Education & Experience:
- Bachelors’ degree with major course work in social work, psychology or another field closely related field. Preferred focus in Social Work
- At least two (2) years of experience in social case work, eligibility determination, social service or community resource referral, or closely related work.
- Ability to communicate effectively with a diverse audience.
- Knowledge of general interviewing practices and/or techniques; knowledge of social work and counseling practices and techniques, including crisis prevention and intervention.
- Must have experience with Microsoft Office Suite & ability to work with online platforms and patient portals.
Environment & Physical Demands: The environment is climate controlled (i.e., heated and air-conditioned). Lighting is adequate through overhead lighting, as well as windows and blinds. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
At times, social worker to make face to face assessments and follow up with members.
Physical demands include sitting and/or standing for extended periods of time, bending, lifting approximately 25 pounds, regular use of the telephone, copier, fax and computer. Demands also involve protracted concentration.

Requirements:
Bachelors’ degree with major course work in social work, psychology or another field closely related field. Preferred focus in Social Work
- At least two (2) years of experience in social case work, counseling, eligibility determination, social service or community resource referral, or closely related work.
·Previous experience in community-based case management or related field highly preferred.
·Must be proficient in Microsoft Office Suite (Word, Excel, Outlook).
·Must be able to pass a criminal background check and drug test.Duties:
- Provide comprehensive social work services to individuals and families in need
- Conduct assessments to determine client needs and develop appropriate intervention plans
- Collaborate with other professionals and agencies to coordinate services and resources
- Advocate for clients' rights and access to necessary support systems
- Provide counseling and support to individuals and families dealing with various challenges
- Assist with childcare and child welfare services, ensuring the safety and well-being of children
- Support workforce development initiatives by providing career counseling and job placement assistance
- Assist clients with accessing Medicare benefits and navigating the healthcare system
- Supervise and monitor clients' progress towards achieving their goals
- Provide mentoring and guidance to individuals in need of additional support
- Utilize motivational interviewing techniques to empower clients and promote positive change
Requirements:
- Bachelor's or Master's degree in Social Work or a related field
- State licensure or certification as a Social Worker (if applicable)
- Knowledge of child welfare policies and procedures
- Familiarity with Section 8 housing programs and regulations
- Strong communication skills, both written and verbal
- Ability to work effectively with diverse populations
- Excellent problem-solving and decision-making abilities
- Empathy, compassion, and a genuine desire to help others
