In our physician-led care model, medical assistants (or patient navigators) work closely with physicians and other members of the team to implement the care plan. Because we care for patients who have difficulty getting to the doctor, the work will be either in the patient's home or remotely from your home. What you will do?
- Support the clinician in preparing the patient for the visit, which may include “virtual” rooming, conducting a home visit prior to the physician, or accompanying the physician for an in-home visit
- Contact patients to collect clinical and psycho-social information
- May conduct new patient intake, which includes an initial home assessment to prepare for in-person visits by the clinician, mitigate fall risk, teach infection control measures, etc.
- Coaches or trains patients on how to utilize technology (e.g., tablets) to communicate with the care team and for virtual visits
- Requests and aggregates patient’s medical records, and updates patient histories
- As directed by the provider, submits referrals, authorizations and medication refill orders, and follows up with the patient accordingly
- Obtains vital signs, chief complaints, and review medications
- Delivers patient screenings per protocols
- Orders supplies and stocks exam rooms
- Conducts routine quality control checks, including infection control measures, equipment QAs, and checks for expired medication and supplies
- Under direction of a Provider, offer MA appropriate “visits”, including administering vaccinations, performing procedures including but not limited to: ABI, spirometry, EKGs, etc.
- Performs waived laboratory testing
- Schedules diagnostic testing
- Applies or assists with the application of durable medical equipment
- Participates in care team meetings to discuss patient care and clinic operations
- Other duties as assigned
What we are looking for?
- Experience delivering clinical care, preferably in an out-patient or in-home setting
- Prior experience working with older adults (65+) is strongly preferred
- Active Medical Assistant certification
- Basic proficiency in MS office products
- Experience using Athena is preferred
- Thrives in a fast paced, ever changing environment, and excited to help build a great organization
- Comfortable with ambiguity and can work independently
- Fluency in Spanish is highly desirable
- US work authorization
- Valid Drivers License, 2 years of driving experience and transportation to facilitate mobile work
- Comfortable going to a patient's home as needed and with little to no notice
- Able to work from home

Pay: $23.00 - $30.00 per hour
